Objective:To evaluate the value of 3D-US-VPQ technology in predicting the risk of ischemic stroke events.Methods:From June 2018 to January 2019,93 patients with carotid plaques in our hospital were enrolled in our research.48 carotid plaques in 41 patients who were clinically diagnosed as acute ischemic stroke were regarded as ischemic cerebral infarction group,their head CT or MRI showed that the infarction was in the same side as carotid plaque.And there were 43 plaques in 40 patients without ischemic stroke or whose head CT or MRI showed that the responsibility of infarction and carotid plaque was not on the same side regarded as non-cerebral infarction group(the control group).All the participants were examined with two-dimensional and three-dimensional carotid ultrasound.A series of carotid atherosclerotic plaque and arterial morphological parameters were obtained by Q App VPQ software.We used SPSS 19.0 software to compare and analyze the difference between two groups and applied ROC curve to compare the efficiency of the above parameters for predicting the risk of ischemic cerebral infarction.Results:1.The mean value of GSM(47.53±14.94)in the cerebral infarction group was significantly lower than that in the non-cerebral infarction group(111.54±33.27)and the difference was statistically significant(P=0.000).The mean values of NWI in the cerebral infarction group(51.53)and WA(50.53)were significantly higher than that in the control group(38.61,39.75),and the differences were statistically significant(P<0.05).There were no differences in LA,TPV,PT and PA between two groups(P>0.05).2.GSM: ROC curve AUC = 0.992,95% CI 0.981-1.000,P = 0.000.When plaque GSM = 69.5,sensitivity = 0.976,specificity = 0.937.NWI: ROC curve AUC=0.644.WA: ROC curve AUC=0.620Conclusion:1.The lower the GSM,the higher the risk of plaque rupture.The higher the NWI and WA,the heavier the plaque load,and the more likely that the rupture leads to the occurrence of ischemic stroke.And GSM has the highest diagnostic efficacy for those who have mild carotid artery stenosis.2.The risk of stroke is higher when GSM<69.5;3.3D-US-VPQ technology can assess the vulnerability of carotid plaque and predict the risk of ischemic stroke events.
