| ObjectiveMindfulness intervention was applied to elderly patients with type 2 diabetes mellitus complicated with depression in community.The intervention effects on depression,emotional distress,quality of life and related biochemical indicators were objectively analyzed through the effect evaluation,which provided some reference for improving the mental health level of elderly patients with type 2 diabetes mellitus complicated with depression in community.MethodsForty-five elderly patients with type 2 diabetes mellitus complicated with depression were selected from A and B communities in Chengdu,totaling 90 cases.The former serves as the experimental group and carries out mindfulness intervention,while the latter serves as the control group and carries out routine psychological nursing.The intervention of the two groups was carried out on the basis of community routine health education for diabetes mellitus.The duration of intervention was 10 weeks.Before and after the intervention,blood glucose,depression,emotional distress and quality of life were compared between the two groups.All data were analyzed and processed by SPSS21.0 statistical software,P<0.05 is considered that the results had statistical significance.ResultsA total of 85 elderly patients with type 2 diabetes mellitus complicated with depression were retrieved,including 42 cases in the experimental group and 43 cases in the control group.The specific results are as follows:1.Comparison of general conditionsBefore the intervention,the general information,blood glucose level(fasting blood glucose,2-hour postprandial blood glucose),depression severity index,score of diabetes-related problems,score of diabetes-related quality of life and D-type personality questionnaire were compared between the two groups,and there was no significant difference(P>0.05).There was Comparability between two groups of patients.2.Comparison of depressionAfter the intervention,the SDS scores of the two groups were compared.The results showed that the SDS scores of the experimental group were lower than those of the control group(P<0.05).The results of intra-group comparison showed that the scores of SDS scale in the experimental group and the control group after intervention were lower than those before intervention(P<0.05).3.Comparison of blood glucose levelAfter the intervention,the blood glucose values of the two groups were compared.The results showed that the fasting blood glucose and two-hour postprandial blood glucose values of the experimental group were lower than those of the control group(P<0.05);the fasting blood glucose and two-hour postprandial blood glucose in the experimental group were lower than those before intervention(P<0.05),while the fasting blood glucose and two-hour postprandial blood glucose in the control group were not significantly different from those before intervention(P>0.05).4.Comparison of emotional painAfter the intervention,the scores of diabetes scale were compared between the two groups,and the emotional distress of the patients was analyzed.The results showed that the score of the Diabetes Questionnaire in the experimental group was lower than that in the control group(P<0.05);the scores of Diabetes Questionnaire of the experimental group were lower than those before intervention(P<0.05),and there was no significant difference in the scores of diabetes scale before and after intervention in the control group(P>0.05).5.Comparison of quality of lifeAfter the intervention,the scores of Diabetes Specific Quality of Life in the two groups were compared.The results showed that the total score,physiological function,psychological function and social relationship of diabetic patients in the experimental group were lower than those in the control group(P<0.05).There was no significant difference in the scores of therapeutic dimension between the two groups(P>0.05).The total score and each dimension score in the experimental group were all lower than those before intervention(P<0.05),while there was no significant difference in the total score and each dimension score before and after intervention in the control group(P>0.05)6.Comparison of type D personality statusAfter the intervention,the scores of type D personality questionnaire were compared between the two groups.The results showed that the scores of negative emotional dimension in the experimental group were lower than those in the control group(P<0.05)while there was no significant difference in the scores of social inhibition dimension between the two groups(P>0.05);After intervention,the scores of negative emotional dimension in the experimental group were lower than those before intervention(P<0.05),and there was no significant difference in the scores of social inhibition dimension before and after intervention(P>0.05);There was no significant difference in the scores of negative emotional dimension and social inhibition dimension in the control group before and after intervention(P>0.05)Conclusions1.Mindfulness intervention can reduce patients’fasting blood sugar and 2-hour postprandial blood sugar,alleviate depression,reduce emotional distress,improve quality of life,improve D-personality to a certain extent,and have a good regulating effect on the elderly’s physical and mental state2.Mindfulness intervention can be extended to patients with diabetes mellitus and depression in the community.Later,mindfulness intervention can be gradually extended to the treatment and nursing of patients with other chronic diseases and negative emotions in the community.It can improve the rehabilitation and management level of chronic diseases in the community and promote the physical and mental health of the elderly with chronic diseases in the community. |
