With the improvement of people’s living standards,more and more parents begin to pay attention to the height of children.There are many diseases of short stature in children,among which growth hormone deficiency(GHD)and idiopathic short stature(ISS)are the most common.Recombinant human growth hormone(rhGH)has significant effect on improving the height of children.Some children have abnormal lipid metabolism and accumulation in addition to their physical development.Literature shows that GH can not only significantly improve the growth of short children,but also promote the decomposition of lipids.At the same time,more and more scholars explore the effect of rhGH on cardiovascular diseases by observing the changes of lipid metabolism before and after replacement therapy.However,the results of the study are still inconclusive.To this end,this study selected children with GHD and ISS who were treated regularly and followed up regularly in the pediatric outpatient department of Chengde Medical College,and observed the effect of rhGH treatment on the efficacy,BMI and blood lipid of the two children,and explored whether rhGH can influence the incidence of cardiovascular diseases.Objective:1.After one year of rhGH treatment,the changes of GHD,ISS physical development indexes(Ht,HtSDS,GV)were observed and the curative effect of rhGH was evaluated.2 To observe the changes of BMI,AI,TCH,TG,HDL-C and LDL-C after rhGH treatment for one year,and to understand the effects of rhGH on lipid metabolism and cardiovascular diseases.Methods:From January 2014 to January 2018,there were 50 children with GHD(18 males and 32 females),with an average age of 8.62 ± 2.80 years,45 children with ISS(33 males and 12 females),with an average age of 7.22± 2.43 years.All children were pre-adolescent children.The initial treatment dose of GHD children is 0.10IU/(kg.d)and ISS children is 0.15IU/(kg.d).The dosage can be adjusted according to the actual situation.During the treatment period,regular follow-up visits were conducted in our outpatient clinic every 3 months.The follow-up included physical examination and the detection of blood lipid related indicators.Excel software is used for data recording and SPSS19.0 software is used for data processing.Measurement data were expressed as mean ±standard deviation.The paired t-test was used to compare the data of normal distribution before and after treatment for the first year;the single factor analysis of variance was used for the comparison of multiple groups before and after treatment;and the independent sample t-test was used for the comparison between groups.The difference was statistically significant with P < 0.05.Results:1.The GV and HtSDS of children with GHD after 1 year treatment were higher than those before treatment,and the difference was statistically significant(P < 0.05).2.After 9 and 12 months of treatment,the Ht of children with GHD was higher than that before treatment and BMI was lower than that before treatment,with statistical significance(P<0.05).There was no significant change in Wt before and after treatment(P > 0.05).3.After 6,9 and 12 months of GHD treatment,HDL-C was significantly higher than that before treatment,LDL-C was significantly lower than that before treatment(P<0.05);TCH and TG were not significantly different before and after treatment(P>0.05);AI was significantly lower after 6,9 and 12 months of treatment than that before treatment,and 12 months of treatment was lower than that after 3 months of treatment(P < 0.05).4.The GV and HtSDS of ISS children after 1 year treatment were higher than those before treatment,and the difference was statistically significant(P < 0.05).5.The Ht and BMI of children with ISS after 9 and 12 months of treatment were significantly higher than those before treatment and BMI was significantly lower than those before treatment(P < 0.05).The Ht of children after 12 months of treatment were significantly higher than those after 3 months of treatment(P< 0.05).There was no significant change in Wt before and after treatment(P > 0.05).6.After 6,9 and 12 months of ISS treatment,HDL-C was significantly higher than that before treatment,and LDL-C was significantly lower than that before treatment(P < 0.05);TCH and TG had no significant difference before and after treatment(P > 0.05);AI had a gradual downward trend,but there was no significant difference(P > 0.05).7.The results showed that GV,△HtSDS,△HDL-C of GHD children after one year of treatment had more significant changes than ISS children(P > 0.05).Conclusion:1.RhGH treatment can significantly improve the growth of GHD,ISS children with Ht,HtSDS,GV,BMI,and the effect is satisfactory,and GHD group HtSDS,GV changes are more obvious,indicating that rhGH on GHD children with growth promotion effect is better than ISS children.3.HDL-C,LDL-C and AI in children with GHD were significantly improved,HDL-C and LDL-C in children with ISS were improved,and AI had a downward trend,indicating that exogenous rhGH might reduce the risk of long-term cardiovascular disease.
