Objective Thyroid nodule is a very common endocrine system disease.In recent years,with the gradual popularization of health checkups and the widespread use of high-resolution ultrasound,the detection rate has been increasing.Although most thyroid nodules are benign,5%-15%of nodules may be malignant.Thyroid cancer is the most common malignant tumor of the endocrine system.In the past 20 years,its incidence has shown an increasing trend on a global scale.Studies have shown that the occurrence of thyroid nodule is closely related to metabolic syndrome.To further investigate the association between benign and malignant thyroid nodules and components of metabolic syndrome,we collected clinical data from 638patients with benign thyroid nodules and 390 patients with thyroid malignant nodules,and performed statistical analysis,in order to identify high-risk groups of thyroid nodules in clinical diagnosis and treatment,provide more accurate identification of benign and malignant,early detection of malignant nodules to improve prognosis.Methods We collected clinical data from patients who underwent surgery for thyroid nodules in Linyi People’s Hospital from January 2015 to December 2017.A total of 1028patients were included in the study.According to the postoperative pathological results,they were divided into benign nodules group and malignant nodules group,including 638 in the benign group and 390 in the malignant group.The survey included gender,age,height,weight,fasting plasma glucose(FPG),blood pressure,triglyceride(TG),total cholesterol(TC),low-density lipoprotein cholesterol(LDL-C),high-density lipoprotein cholesterol(HDL-C),blood uric acid,thyroid stimulating hormone(TSH),free thyroxine(FT4),free triiodothyronine(FT3)and thyroid peroxidase antibody(TPOAb).At the same time,the results of postoperative pathological results were recorded.We performed a statistical analysis of each survey item to explore the relationship between the benign and malignant thyroid nodules and the components of the metabolic syndrome.Results 1.Among 1028 surgical patients,638 were benign nodules,accounting for 62.06%;390 were malignant nodules,accounting for 37.94%.The mean age of malignant group was lower than that of benign group(P<0.001).According to different age groups,patients aging30-69 years,the proportion of malignant thyroid nodules decreased with age.There was no significant difference in the ratio of male and female between the benign group and the malignant group(P>0.05).2.The level of BMI in the malignant nodules group was higher than that in the benign nodules group(P<0.001).According to different BMI stratification,the obese group had the highest proportion of malignant thyroid nodules,followed by super-recombination.3.The level of FPG in the malignant nodules group was higher than that in the benign nodules group(P<0.001).According to different FPG stratification,the proportion of thyroid malignant nodules was the highest in the group with impaired fasting glucose regulation,followed by the diabetic group.4.The level of HDL-C in the malignant nodules group was lower than that in the benign nodules group(P<0.05).There was no significant difference in the levels of TC,TG and LDL-C between the two groups.5.There was no significant difference in blood pressure and blood uric acid between benign nodules group and malignant nodules group(P>0.05).6.The level of TSH in the malignant nodules group was higher than that in the benign nodules group(P<0.001).The stratified comparison was performed according to different TSH levels.When TSH increased,the proportion of thyroid malignant nodules also increased.The levels of FT3 and FT4 in malignant nodules group were lower than those in benign nodules group(P<0.05).The positive rate of TPOAb in malignant nodules group was higher than that in benign nodules group(P<0.001).7.Multivariate logistic regression analysis showed that TPOAb,FPG,TSH,BMI and age were risk factors for malignant thyroid nodules.Among them,the occurrence of thyroid cancer was negatively correlated with age(OR value 0.969),and positively correlated with positive TPOAb,FPG,TSH,BMI(OR values were 2.696,1.435,1.195,1.141,respectively).Conclusion 1.The levels of BMI,FPG and TSH in malignant nodules group were significantly higher than those in benign nodules group.2.The levels of age and HDL-C in malignant nodules group were significantly lower than those in benign nodules group.3.The positive rate of TPOAb in the malignant nodules group was significantly higher than that in the benign nodules group.4.There were no significant differences among blood pressure,TC,TG,LDL-C and blood uric acid levels in benign and malignant nodules groups.5.The occurrence of thyroid cancer was negatively correlated with age,and positively correlated with positive TPOAb,FPG,TSH and BMI.
