| 【Objective】To explore the occurrence of post-traumatic stress disorder(PTSD)and its influencing factors in patients with traffic accidents in different periods,and to provide theoretical basis for psychological intervention and treatment of PTSD in the future.【Methods】In this study,we selected the first affiliated Hospital of Hainan Medical University from November 2017 to June 2018,the Second Affiliated Hospital of Hainan Medical University,the Hainan Hospital of Hainan Medical University,and the Emergency Trauma surgery Department of Haikou people’s Hospital.A follow-up survey was conducted in 313 traffic accident patients admitted to th e Trauma Medical Center,Orthopaedics,General surgery,Neurosurgery and Oral and Maxillofacial surgery.306 patients were involved in the whole process.General data questionnaire,Post-traumatic stress Disorder scale(PCL-C),Hamilto n anxiety scale(HAMA),Hamilton Depression scale(HAMD),simple coping style questionnaire(SCSQ),were used.The Social support rating scale(SSRS)was investigated and evaluated one month after the accident,three months and six months after the accident.【Results】⑴306 cases of traffic accident injury were included in this study.The average age was 43.62 ±16.23 years old,of which 56.9% were male,43.1% were female,Han nationality 73.9%,61.1% were married,and 77.4% were from senior high school and below.⑵The incidence of(post traumatic stress disorder,PTSD)was 33%,37.6% and 40.5% respectively at one month,three months and six months after road traffic accident.After two follow-up,patients with PTSD caused by traffic accident each time increased the number of cases on the basis of the previous round of investigation.⑶The main influence factors of post-traumatic stress disorder are as follw sex(OR=14.75,95%CI=4.802~45.33),personality(OR=1.527,95%CI=0.196~3.542),resident population of migratory birds in other provinces(OR=4.283,95%CI=3.695~7.234).The place of residence is the foreign provincial migrant worker s(OR=3.170,95%CI=2.364~4.981),the resident place is the provincial tourist p opulation(OR=1.629,95%CI=1.058~2.510),the source of medical expenses(OR=3.025,95%CI=1.382~6.943),Poor relationship with family(OR=13.99,95%CI=4.841~40.41),injury to relatives in traffic accidents(OR=1.798,95%CI=2.035~6.138),negative coping(OR=1.121,95%CI=0.891~1.625),subjective support(OR=0.473,95%CI=0.273~0.818),Objective support(OR=0.444,95%CI=0.245~0.806),cognitive disorder(OR=2.037,95%CI=1.117~3.718),block symptoms(OR=2.701,95%CI=1.343~5.434),total depression score(OR=1.251,95%CI=1.126~1.390),Physical anxiety(OR=2.104,95%CI=1.442~3.068),mental anxiety(OR=2.015,95%CI=1.330~3.052),total anxiety score(OR=1.735,95%CI=1.371~2.196),trauma severity(OR=1.967,95%CI=1.467~2.639).【Conclusions】⑴The probability of PTSD in a patient with a car accident was high and the rate of PTSD increased gradually over time.⑵Post-traumatic stress disorder has the characteristics of delay,persistence and long-term.⑶Gender,personality,residence,source of medical expenses,relationship with family,family injury in traffic accidents,coping style,social support,depr ession,anxiety and trauma severity were the influencing factors of PTSD.Among them,subjective support and objective support are the protective factors ofpost-traumatic stress disorder.Poor relationship with family,injury of relatives,negative coping,anxiety and depression are the risk factors of post-traumatic stress disorder in traffic accidents.It is suggested that medical staff should screen the risk factors and occurrence of ptsd as soon as possible,give targeted psychological guidance and extensive social support to the victims of car accid ent injury,and guide them to deal with the accident injury correctly,so as to promote the patients’ physical and mental health. |
