Objective:This clinical study is to observe the clinical efficacy and adverse reactions of acupuncture combined with cauterization treatment of chronic tonsillitis,to explore its therapeutic mechanism, and to evaluate the efficacy and safety of this therapy. The purpose of this study is to provide a practical and effective standardized treatment scheme for clinical diagnosis and treatment of chronic tonsillitis, reflecting the characteristics of traditional Chinese medicine,so as to promote its application throughout the country.Methods:60 eligible subjects were randomly divided into three groups: acupuncture group, cauterization group and acupuncture combined with cauterization group. The general data and biological distribution of all subjects were balanced, with no statistical significance and comparability. In the acupuncture combined with cauterization group, the blood was released from bilateral tonsils, and each tonsil was cut for 5 times. Then the tonsils were cautered. The tonsils were heated with metal cautery and cautered. Each tonsil was cautered for 5 times, and then sprayed with tin powder on the affected area.Needle-knife puncture therapy was used in the acupuncture group: blood was released by puncturing bilateral tonsils, five times per tonsil, and then tin was sprayed on the affected area.In the cauterization group, the tonsils were heated with metal cautery iron and cautered for 5 times on each side, then tin was sprayed on the affected area. All three groups were treated once every 7 days.Five consecutive treatments were a course of treatment. Before treatment,1 week,3 weeks and 5 weeks after treatment, the time points were observed and recorded. The changes of main and secondary symptom scores and signs were observed before and after treatment in three groups.Result:Before and after treatment, there were significant differences in the scores of primary and secondary symptoms and signs between the three groups, indicating that the three therapies had different degrees of efficacy. The clinical effect of acupuncture combined with cauterization on chronic tonsillitis is better than acupuncture group and cauterization group, but in the short term, there is little difference between combined therapy of acupuncture and cauterization, and long-term combined therapy of acupuncture and cauterization is better than that of cauterization. The treatment of chronic tonsillitis by cauterization is always superior to that by acupuncture therapy in both short-term and long-term efficacy. There were no adverse reactions such as needle fainting, hemorrhage, infection and pain before and after treatment in the three groups. The safety evaluation of all cases in this trial was grade 1.Conclusion:The mechanism of acupuncture combined with cauterization therapy for chronic tonsillitis is to clear channels and collaterals, discharge blood and pus, release heat and toxicity, and achieve the effect of detumescence, dissipation of blood stasis, dispelling pathogenic factors and strengthening the body. This therapy has obvious curative effect on chronic tonsillitis. It is simple, safe and reliable, less trauma,no recurrence, low price and no toxic side effects. It is worthy of further promotion and application in clinic.
