| Objective:To explore of epidural labor analgesia effects on S100βprotein level of umbilical blood.Methods:According to the established exclusion and inclusion criteria and the principle of voluntary participation in this study,primary parturients who voluntarily accepted epidural labor analgesia in line with the inclusion criteria were divided into group E,and those who voluntarily gave up labor analgesia in line with the inclusion criteria were divided into group S,(n=50 each).Group E:When the cervical dilation was opened to 3cm,the epidural puncture was performed in the L2-3 intervertebral space.An epidural catheter was placed at the head end,and the experimental dose of 1%lidocaine was injected,followed by an observation for 3 to 5 minutes to excluding the catheter from the vascular or subarachnoid space,and then 8-12ml of the mixture of 0.1%ropivacaine with 0.25ug/ml sufentanil was injected as the first dose to obtain a satisfactory effect followed by PCA pump.The background infusion rate is 8-10ml/h,PCEA volume 3-5ml,locking time 20 minutes,the maximum amount of 20ml per hour,fetal placenta delivered,suture wounds completed followed by the withdrawal of the tube.Monitoring maternal respiration,blood pressure and SpO2 during the whole process to keep them within the normal range;Group S:according to the conventional natural delivery procedures.The VAS scores of maternal cervix at3,6,8,and 10 cm,the Apgar scores at 1 min and 5 min after birth,the first stage of labor,the second stage of labor,neonatal weight,the umbilical arterial and umbilical venous blood gas analysis were recorded in the two groups;Two groups of umbilical arterial and umbilical venous blood(2ml each)collected by centrifugation was separated and stored in the refrigerator of-20 degrees Celsius.After all samples were collected,the concentration of S100βprotein levels in plasma were determined with enzyme-linked immunoassay(ELISA).SPSS 22.0 was used for statistical analysis.Results:Due to insufficient umbilical cord blood collection and failure of plasma separation,only 92 people completed all the tests.including 47 in E group and 45 in S group.The VAS score in the E group after labor analgesia was significantly lower than that in the S group(P<0.05).There were no significant difference in Apgar score,the labor process,neonatal weight,the umbilical arterial and umbilical venous blood gas analysis,BE value,serum lactate concentration,umbilical arterial blood S100βprotein(UA),umbilical venous blood S100βprotein(UV)and(UA-UV)between the two groups after birth.Conclusion:Epidural labor analgesia had no effect on Apgar score,the umbilical arterial and umbilical venous blood gas analysis,BE value,serum lactate concentration,S100βprotein level in cord blood and neonatal oxygenation.Patient-controlled epidural labor analgesia from the active stage of the first labor process would not affect the labor process.Meanwhile,the analgesic effect of epidural labor analgesia is exact.It is a safe and effective method of labor analgesia. |
