Association Between Infarct Volume And CD147, Blood Routines, Lipids Or Fibrinogen Of Patients With Acute Ischemic Stroke

Objective: This study aimed to investigate the correlation and association between infarct volume and CD147,blood routines,serum lipids,or fibrinogen.Methods: Patients with acute ischemic stroke or transient ischemic stroke(TIA)admitted to the Neurology Department of The Second Hospital of Hebei Medical University were prospectively and consecutively recruited from December 2017 to October 2018.Fasting blood samples were collected on the next morning after admission,and CD147,blood routines,lipids and fibrinogen were tested.All patients in our study underwent brain CT scans within 24 hours and brain 3.0T MRI scan include DWI series within the first 3 days after admission.Patients were divided into 2 groups,infarction group and TIA group,according to whether there were ischemic lesions on DWI.And the infarct volume was measured and calculated using DWI.Then we analyzed the relationship between those risk factors and infarct volume.Results: A total of 137 eligible patients were enrolled in this study,including 114 patients with cerebral infarction and 23 with TIA.Their baseline clinical information did not show statistically significant difference.(1)The level of CD147 did not show any significant difference among groups with different infarct volume.(2)Spearman correlation analysis showed that total cholesterol(r=0.253,P=0.007),LDL-C(r=0.263,P=0.005),apoB(r=0.187,P=0.047),fibrinogen(r=0.197,P=0.036)and LDL-C/HDL-C ratio(r=0.211,P=0.024)were significantly and positively correlated with infarct volume.(3)Ordinal logistic regression showed that hypertension(P=0.031,OR=2.298,95%CI 1.077-4.904),LDL-C(P=0.008,OR=1.521,95%CI 1.113-2.079)and fibrinogen(P=0.025,OR=1.832,95%CI 1.079-3.109)were independently associated with infarct volume.(4)ROC curves showed that neutrophil/lymphocyte ratio had diagnostic value for ischemic stroke(AUC=0.631,P=0.049)and the cutoff value was 2.370.Conclusion: Our data suggested that the level of serum CD147 was not associated with infarct volume.However,total cholesterol,LDL-C,apoB,fibrinogen and LDL-C/HDL-C ratio were positively correlated with infarct volume.Hypertension,LDL-C and fibrinogen were independently associated with infarct volume.And neutrophil/ lymphocyte ratio had diagnostic value for ischemic stroke.
