Objective: To explore the effect of motivational interviewing on self-management level of maintenance hemodialysis patients,in order to stimulate the intrinsic motivation of patients’ behavior change,and promote the change of their behavior and lifestyle,in order to improve the self-management level of patients and the adequacy of dialysis,improve the quality of life of patients,and provide reference for health education of hemodialysis patients.Method: This research was belonged to experimental research.Maintenance hemodialysis patients in a tertiary first-class hospital were selected as the research subjects and randomly divided into control group(46)and experimental group(46).The control group was given health education by routine method,while the experimental group was given health education by motivational interviewing method.The contents of health education and the intervening time were the same in the two groups.The contents of health education include diet management,medication management,exercise,pipeline maintenance,condition monitoring and psychological nursing.The intervening time is 6 months.Before and after intervention,the self-management level,adequacy of dialysis and quality of life of patients were evaluated and analyzed.Self-management level was assessed by the Self-management Scale for Hemodialysis Patients.Urea decline score(URR%)and urea clearance index(KT/V)were used to assess dialysis adequacy.Quality of life was assessed by the WHOQOL-BREF.Result: 1 General situation of patients The demographic and disease-related aspects such as sex,age,dialysis time,marital status and educational level between the control group and the experimental group were compared.The results showed no significant difference(P > 0.05),and were comparable.2 Self-management level,dialysis adequacy and quality of life of subjects before intervention Before intervention,the total score of self-management in the experimental group was(46.80±11.23),and that in the control group was(46.96±8.77).There was no statistical difference in the total score of self-management and the scores of each dimension between the two groups(P > 0.05).Before intervention,the URR and KT/V values of the experimental group were(64.68±7.92)and(1.36±0.24)respectively,while the URR and KT/V values of the control group were(64.12±7.65)and(1.30±0.20),respectively.There was no significant difference in the URR and KT/V values between the two groups(P > 0.05).Before intervention,the total score of QOL in the experimental group was(67.61±11.33)and that in the control group was(68.89±14.25).There was no significant difference in the total scores of QOL and the scores in various fields between the two groups(P > 0.05).3 Intervention effect of control group After intervention,the total scores and dimensions of self-management of patients in the control group were higher than those before intervention,and the difference was statistically significant(P < 0.05).After intervention,the URR and KT/V values of patients in the control group were higher than those before intervention,and the difference was statistically significant(P < 0.05).After the intervention,the total scores of QOL and the scores of each field in the control group were higher than those before the intervention,and the difference was statistically significant(P < 0.05).4 Intervention effect of experimental group After intervention,the total scores of self-management and the scores of each dimension in the experimental group were significantly higher than those before intervention(P < 0.01).After intervention,the URR and KT/V values of the experimental group were significantly higher than those before intervention(P < 0.01).After intervention,the total scores of QOL and the scores of various fields in the experimental group were significantly higher than those before intervention,and the difference was statistically significant(P < 0.01).5 Comparison of intervention effect between experimental group and control group The total scores and dimensions of self-management in the experimental group were significantly higher than those in the control group,and the difference between the two groups was statistically significant(P < 0.01).The number of URR and KT/V in the experimental group was significantly higher than that in the control group,and there was a significant difference between the two groups(P < 0.05).The total scores of QOL and the scores in various fields in the experimental group were significantly higher than those in the control group,and the difference between the two groups was statistically significant(P < 0.01).Conclusion: 1 The self-management level of maintenance hemodialysis patients is low,the dialysis adequacy is poor,and the quality of life is low.2 Motivational interviewing health education and Routine health education can improve the self-management level and dialysis adequacy of maintenance hemodialysis patients,and improve their quality of life.3 Motivational interviewing health education is better than routine health education.Motivational interviewing is an effective health education method,which is worth promoting.
