A Practical Study Of Pharmacists Participating In The Safety Of Medication For Special Populations Based On Safe Pharmacist System

OBJECTIVE Explore the role of safety pharmacists participate in the safety of medication for special population.METHODS Establish a safety pharmacist system,monitor the treatment process of special population through safety pharmacists.Used retrospective study to analysis the two cases side effects of special populations involved in consultation and monitoring by safety pharmacists.Summarized physiological changes and medication-related problems in special populations,and explored the role of safe pharmacists in drug safety in special populations.RESULTS Elderly patients had more basic diseases,poor health condition.They suffer from multiple diseases at the same time,and use multiple drugs.Therefore,the medication was prone to generate adverse effects,A new adverse drug reaction induced by tegacycline was found in one elderly patient.Pregnant women had special physiological condition,the number of pregnant women with chronic diseases increases during pregnancy.Both doctors and pregnant women attach great importance to drug safety.After aztreonam causes allergic reactions in pregnant women,the safety pharmacist monitor the whole process and give drug education to the patient until the patient is discharged from hospital.Safety pharmacistsfollowed with clinical pharmacists to participate in the whole process of diagnosis and treatment in special population,and follow-up to complete the pharmacy service.Conducive to ensuring the safety of medication for special populations during hospitalization.CONCLUSION Through the establishment of the safety pharmacist system,safety pharmacists are involved in the diagnosis and treatment of special populations,participate in the identification of adverse effects,pay attention to the characteristics of adverse drug reactions in special population,pay attention to the outcome of adverse drug reactions,and continue to follow up on pharmacy services,which will play an important role in ensuring the safety of medication for special populations.
