Objective: To retrospectively analyze the clinical data of patients with hepatitis complicated with acute pancreatitis and to understand the clinical features and prognosis of various types of hepatitis complicated with acute pancreatitis.Methods: The clinical data of 63 patients with acute pancreatitis admitted to our hospital from January 2011 to January 2019 were retrospectively analyzed,summarizing the clinical features and prognostic factors of hepatitis complicated with acute pancreatitis.Results: 1.General information: There are 63 patients who were diagnosed with hepatitis complicated with acute pancreatitis,among which were 49 males and 14 females,aged 21 to 69 years,with a median age of 45 years.2.Etiology and clinical type:(1)Among the 63 cases,there were 59 cases of viral hepatitis complicated with acute pancreatitis,accounting for 93.6%(59/63),including 1 case of hepatitis A,52 cases of hepatitis B,and 1 case of hepatitis A combined with hepatitis B,1 case of hepatitis B combined with hepatitis E,1case of hepatitis C virus,3 cases of hepatitis E virus.There was 1 case of autoimmune hepatitis,accounting for 1.6%(1/63).There were 3 cases of unexplained liver damage,accounting for 4.8%(3/63).(2)Among the 63 cases,6 cases were acute hepatitis,accounting for 9.5%(6/63),all of which were acute jaundice,15 cases were chronic hepatitis,accounting for 23.8%(15/63),including 2 cases of moderate hepatitis and 13 cases of severe hepatitis,6 cases were cirrhosis,accounting for 9.5%(6/63),36 cases were liver failure,accounting for 57.1%(36/63),including 1 case of acute liver failure,2 case of subacute liver failure,21 cases of acute-on-chronic liver failure and 12 cases of chronic liver failure.3.Symptoms and signs: Among the 63 patients,31 cases had abdominal pain symptoms,32 cases had nausea and/or vomiting,3 cases had fever,among which all were low fever,only 1 case were at 39 °C.There was 1 case of diarrhea,12 patients had no obvious abdominal pain,nausea,vomiting and fever,and they were only diagnosed by routine abdominal Doppler ultrasound and blood lipase amylase.3 patients had muscle tension,36 patients had abdominal tenderness,20 patients had rebound tenderness,29 patients had no obvious abdominal signs,and 6 patients had neither symptoms nor signs of acute pancreatitis.4.Prognosis 28 patients died or went worse,and 35 patients were discharged after hospitalization.Of the 63 cases,6 cases of acute hepatitis were all discharged from hospital,among all 15 cases of chronic hepatitis,14 cases of recovered and discharged,1 case died or got worse,6 cases of cirrhosis patients all recovered and discharged,Among 36 cases of liver failure patients,9 patients recovered and discharged and the other 27 patients died or got worse,accounting for 75%(27/36).Conclusion: 1.Compared with other types of hepatitis,viral hepatitis(93.6%)is more prone to acute pancreatitis.2.Acute pancreatitis as one of the complications of hepatitis,its incidence is related to the clinical type of hepatitis,patients with liver failure(57.1%)is more likely to be complicated with acute pancreatitis,and the prognosis is worser(75% of patients died or got worse).3.The symptoms and signs of hepatitis complicated with acute pancreatitis are atypical,making it difficult to obtain timely diagnosis and treatment.This delays the patient’s treatment and affects the prognosis.
