Comparison Of Curative Effects Of Three Different Surgical Methods In Treating Angle-closure Glaucoma In Patients With Abnormal Lens Suspensory Ligament

Objective: To investigate the effects of phacoemulsification combined with phacoemulsification,phacoemulsification combined with phacoemulsification and capsular tension ring or phacoemulsification combined with phacoemulsification and vitreous resection of three different surgical methods of clinical curative effect.Methods: From January 2017 to December 2017,a total of 80 patients(80 eyes)with secondary angle closure glaucoma were selected randomly from our hospital and divided into three groups according to three different surgical methods,including mild,moderate and severe group of three groups.The patients in the mild group were treated with micro-phacoemulsification and lens implantation,while those in the moderate group were treated with micro-phacoemulsification,lens implantation and tension-ring implantation,and patients in the severe group were treated with microscopic phacoemulsification,lens suspension and anterior vitrectomy.Three groups of 80 cases of corrected visual acuity,intraocular pressure changes,the central anterior chamber depth and postoperative complications were compared.Result:After 1 year of treatment,corrected visual acuity was improved in 80 patients in three groups during follow-up.Intraocular pressure was lower than before treatment,in which mild intraocular pressure was 9.1 ~ 14.8mmHg,moderate group was 5.2 ~ 16.1mmHg,severe group was 10.2 ~ 18.1mmHg.There was no significant difference in the depth of the central anterior chamber between the mild group,the moderate group and the severe group before treatment.The depth of anterior chamber was 1.82 ~ 3.28 mm in mild group,1.2 ~ 4.1mm in moderate group and 2.2 ~ 4.5mm in severe group.Mild group,moderate group and severe group were significantly better than before treatment,in which severe intraocular pressure in patients with intraocular pressure within 22 mmHg,compared with before treatment,the difference was statistically significant(P <0.05).Three visual acuity was restored to 0.3 to 1.2 range.Of the 18 patients with lens suspension,1 patient had IOL deviation and no significant visual acuity.The mild group and the severe group had 100.00% power.The intraocular pressure of one case in the moderate group was not significantly improved after the operation.The intraocular pressure of the patients with glaucoma drainage threshold was effectively controlled after the drug treatment was invalid.Conclusion: The curative effect of capsular tension ring,lens suspension,anterior vitrectomy combined with phacoemulsification and intraocular lens implantation in the treatment of patients with secondary angle-closure glaucoma with abnormal lens suspensory ligament is superior and has higher safety,it should choose a reasonable surgical approach according to the specific clinical of patients.It is worth clinical promotion and reference.
