Objective 1.To observe the effect of sufentanil combined with ropivacaine in epidural labor analgesia.2.To investigate the appropriate concentration of sufentanil in epidural labor analgesia.Method 120 primipara aged 20-35 years with vaginal delivery in Zhucheng Maternal and Child Health Hospital from March 2018 to June 2018 were selected.According to the concentration of sufentanil,30 cases in each group were randomly divided into four groups: group A with 0.1% ropivacaine,group B with 0.3 ug/ml sufentanil combined with 0.1% ropivacaine hydrochloride,group C with 0.5 ug/ml sufentanil combined with 0.1% ropivacaine hydrochloride,group D with 0.7 ug/ml sufentanil combined with 0.1% ropivacaine hydrochloride.Continuous epidural block was used in all cases.Visual analogue scale(VAS)of pain was observed and recorded before and 10,20 and 30 minutes after analgesia,improved Bromage score before and 30 minutes after analgesia,Apgar score 1 and 5 minutes after birth,oxytocin usage rate,cesarean section conversion rate and adverse reaction rate.Postpartum hemorrhage was measured by weighing method.Result There was no significant difference in age,weight and gestational weeks between the four groups(P > 0.05).(2)The VAS scores of C and D groups were lower than those of A and B groups at 10 and 20 minutes after analgesia(P < 0.05).There was no significant difference between C and D groups before and after analgesia(P > 0.05).(3)There was no significant difference between the improved Bromage score 30 minutes after analgesia and that before analgesia in the four groups(P > 0.05),and there was no difference between the four groups(P > 0.05).(4)In the use rate of oxytocin,there was a significant improvement after analgesia compared with that before analgesia(P < 0.05),but there was no significant difference in cesarean section rate,postpartum hemorrhage and 1 minute and 5 minutes after birth between the four groups(P > 0.05).The incidence of adverse reactions of pruritus and urinary retention in group D was significantly higher than that in group A,B and C(P < 0.05).Conclusion The analgesic effect of sufentanil group was significantly better than that of ropivacaine group.The analgesic effect of 0.5 ug/ml and 0.7 ug/ml sufentanil group was significantly better than that of 0.3 ug/ml sufentanil group,and the onset time was shorter.However,the analgesic effect of 0.7ug/ml sufentanil group was not superior to that of 0.5ug/ml sufentanil group.With the increase of sufentanil concentration,the effect is faster and the analgesic effect is better,but there is a capping effect.The incidence of adverse reactions in 0.7ug/ml sufentanil group was significantly higher than that in the other three groups.The application of sufentanil in epidural labor analgesia has good analgesic effect and quick onset.0.5ug/ml sufentanil combined with 0.1% ropivacaine has good effect and fewer adverse reactions,which is worthy of clinical application.
