Study On The Process Intervention Of Medical Case Social Work In Severe Patients

With the progress of modern society,the medical model has be en transformed and upgraded from a simple biomedical model to a bio-psycho-social integrated medical model,and medical social work has been gradually popularized and developed.In October 2018,th e State Health and Health Commission promulgated the Evaluation Indicators of the Action Plan for Further Improvement of Medical Services(2018-2020).It proposed that medical institutions should es tablish the system of medical social workers and volunteers,and th at the system of medical social workers should be separately listed as the first-level evaluation index of medical services.These measur es provided strong policy support for medical social work and clini cal practice of medical volunteers.Under the background of the current changes in the spectrum of human diseases,there are more and more severe diseases in Chi na.Severe patients are different from ordinary mild patients.For se vere patients,a medical action may only temporarily save their live s,and they may still need a series of medical actions to maintain t heir quality of life.Therefore,medical social workers should follow the characteristics of patients with severe illness,analyze the proble ms and needs faced by patients with severe illness and their familie s in different stages of illness,expand professional services from ho spitals to hospitals,in order to improve the experience and living c onditions of patients with severe illness,improve the service system of medical social work and promote the realization of healthy Chi na strategy.Based on this,this study takes the whole process intervention of medical case social work as the breakthrough point,and takes the severe patients as the service object.Firstly,it theoretically analys es the related concepts,theoretical basis,necessity and feasibility of the whole process intervention of medical social work for the seve re patients.Secondly,it collects the practical data of Changsha X Hospital,and takes the case work as the main intervention method,from the time of admission to the time of residence.The service content and process in the whole process of medical social work in tervention were analyzed in the four stages of hospital period,disch arge day and after discharge.Finally,the service effect of the whol e process intervention was evaluated,and the difficulties encountere d and the countermeasures were analyzed.It can effectively alleviate the pressure and crisis of the clients and their families at different medical stages,and ensure the sustai nability of medical social work intervention by providing rapid inter vention at admission,comprehensive support during hospitalization,i mplementation of the plan of discharge day and the whole process of rehabilitation follow-up after discharge.However,the whole proc ess of intervention in medical social work is still at the exploratory and initial stage.It is necessary to further improve the whole proc ess intervention mode of medical social work by building a coopera tive mechanism of multi-participation,improving the policy environ ment for the development of medical social work,and strengthening the professional guidance for the whole process intervention of me dical social work.
