Clinical Study Of Bushen Antai Prescription Combined With Cyclosporin A In The Treatment Of Unexplained Recurrent Spontaneous Abortion Of Kidney Deficiency Type

Objective:To study the clinical efficacy and safety of Bushen Antai Prescription combined with cyclosporine A in the treatment of unexplained recurrent spontaneous abortion of kidney deficiency type,and to explore its therapeutic mechanism,so as to provide a new idea for clinical treatment of unexplained recurrent spontaneous abortion.Methods:The subjects were 81 cases of recurrent abortion,with kidney deficiency of unknown origin,treated in the outpatient department of traditional Chinese medicine of Jiangxi Maternal and Child Health Care Hospital from January 2017 to March 2018.The subjects were randomly divided into a treatment group?43 cases?and a control group?38 cases?.The treatment group was treated with Bushen Antai Prescription combined with CsA,while the control group was treated with CsA alone.The general clinical characteristics,the success rate of fetal protection and the live birth rate of the two groups were collected.The values of T lymphocyte,serum human chorionic gonadotropin?HCG?and estradiol?E2?were compared before and after treatment.The serum levels of hcg which were measured at 6-9 weeks after successful fetal protection,adverse reactions,liver and kidney function before and after treatment,follow-up of offspring.The database of clinical research information was established,and the data were analyzed by software SPSS23.0.The general clinical characteristics of the two groups were tested by independent sample t test.The success rate of fetal protection and live birth rate were tested by chi-square test.The difference before and after treatment was tested by paired sample t test and the difference between groups was tested by an independent sample t test,with P<0.05 as the significant difference.Result:?1?Contrast the curative effect between the two groups:38 cases in the treatment group succeeded in fetal preservation.3 cases resulted in abortion.The success rate was 93%.25 cases in the control group succeeded in fetal preservation,10 cases resulted in abortion.The success rate was71%.There was a significant difference between the two groups?p<0.05?.Live birth rate:90%in the treatment group and 69%in the control group.There was a significant difference between the two groups?p<0.05?.?2?Comparisons of adverse reactions between the two groups:4 cases of liver function damage were observed in the treatment group and 11 cases in the control group,with a significant difference of?p<0.05?.1 case of skin rash was observed in the treatment group and 1 case of leukopenia was observed in the control group.1 case of gestational hypertension was observed in the treatment group,with no significant difference between the two groups?p>0.05?;no abnormalities or deformities were found in any of the live birthed newborns.?3?T lymphocyte comparison before and after treatment:After treatment,there was a decrease in the CD4 and CD4/CD8 ratio in both groups?p<0.05?after treatment,the ratio of CD4/CD8 in treatment group and control group had statistical difference?p<0.05?;there was no statistical difference before and after treatment?p>0.05?.?4?Comparisons of HCG and E₂ before and after treatment in the two groups:After treatment,the values of HCG and E₂ increased significantly in both groups?p<0.01?.After treatment,the values of HCG in the treatment group were significantly higher than those in the control group?p<0.01?,but there was no significant difference in E₂ between the two groups?p>0.05?.?5?For successful pregnant women,the level of HCG in the treatment group was significantly higher than that in the control group at the 6th,7th,8th and 9th weeks?p<0.01?Conclusions:?1?Bushen Antai Prescription combined with CsA has a better effect on URSA than CsA alone,which is worthy of clinical promotion.?2?Bushen Antai Prescription or CsA can inhibit CD4 cells,improve the immune status of pregnant women,and help to maintain a normal pregnancy.?3?Bushen Antai Prescription or CsA can improve the levels of HCG and E₂,and the combined use of them is more significant to improve the level of HCG.?4?CsA can cause liver function damage,and Bushen Antai Prescription can reduce liver function damage.There were no abnormalities or deformities in any live births in this study,but the safety of CsA during pregnancy still needs to be further evaluated.
