| ObjectiveThis study was designed to investigate the characteristics and the influencing factors of chemotherapy-induced peripheral neuropathy(CIPN)among gynecological cancer patients.Based on literature research and expert consultation,the effect of exercise therapy on CIPN was explored in order to provide practical guidance for medical personnel to alleviate the symptoms in the futureMethods1.A total of 124 gynecological cancer patients who was receiving chemotherapy in a hospital in Tianjin from February 2018 to June 2018 were investigated by purposive sampling method.Sociodemographic and disease-related questionnaire,Chemotherapy-induced Peripheral Neuropathy Assessment,Berg Balance Scale and quality of life questionnaire-core 30 were distributed to patients2.A total of 81 patients with ovarian cancer receiving chemotherapy in a hospital in Tianjin from July 2018 to December 2018 were recruited for our study.Patients from July to September 2018 were assigned to the experimental group(n=32)and patients from October to December 2018 were assigned to the control group(n=34).Routine care was given to the patients in the control group.While patients in the experimental group received exercise therapy which include home aerobic exercise and fine movement training for 6 weeks.The severity of CIPN,the degree of distress,the frequency of symptom occurrence,the quality of life and the balance function of the patients were evaluated before intervention and 6 weeks after intervention respectively.Results1.The most common CIPN symptoms were weakness of arms or legs(79.8%),numbness of finger/hand(66.1%)and discomfort of finger/hand or toe/foot(57.3%)The scores of the degree,distress and frequency of weakness of arm or leg were(3.77±2.51),(3.89±2.52),(4.24±3.10),respectively.The severity of interference with enjoying life,normal housework and exercise,and the scores were(4.43±1.15),(3.97±1.88),(3.90±1.77)2.The CIPN symptom scores of patients with different tumor types,tumor stages,chemotherapy regimen and KPS scores were significantly different(P<0.05)Spearman correlation analysis indicated that he CIPN symptom score was significantly associated with age,time of illness,and current chemotherapy cycle(P<0.05).There were significant differences between the effect of CIPN on ADL with different tumor stages and KPS scores(P<0.05).And a positive correlation was found between age and duration of illness and the effect of CIPN on ADL(P<0.05)3.Compared with patients with Berg score of ">45",the severity,degree,frequency of occurrence of CIPN,total score of CIPN symptom and the effect of CIPN on ADL score were significantly higher in patients with Berg score of "<45"(P<0.05).The physical function,role function,emotional function,cognitive function,social function and the overall quality of life were negatively correlated with the scores of CIPN symptoms and the effect of CIPN on ADL(P<0.05).Fatigue,nausea and vomiting,pain,dyspnea,sleep,loss of appetite,constipation,diarrhea and economic difficulties were positively correlated with the scores of CIPN symptoms and the effect of CIPN on ADL(P<0.05)4.After 6 weeks of exercise therapy,the effect of CIPN on ADL and total score of CIPNAT in the experimental group were significantly lower than that of the control group(P<0.05).The percentage of patients with Berg score<45 in the experimental group was significantly lower than that in the control group(P<0.05),and the scores of physical function,emotional function,social function and overall quality of life in the experimental group were significantly higher than those in the control group(P<0.05).The scores of fatigue and sleep in the experimental group were significantly lower than those in the control group(P<0.05)Conclusion1.The symptoms of CIPN in patients with gynecological cancer were at the middle level,which had a moderate effect on the patient’s ADL2.CIPN has a negative effect on the quality of life and balance function of patients with gynecological cancer.Medical staff should pay attention to the quality of life and balance function of patients with CIPN while paying attention to the disease itself,so as to prevent falls.3.Exercise therapy can improve the symptoms of CIPN and its effect on ADL in patients with ovarian cancer.It is worth practicing and popularizing in clinical practice.We should guide the patients to exercise correctly in order to relieve the symptoms of CIPN and improve the quality of life. |
