Objective1.To understand the current situation of chemotherapy-induced peripheral neuropathy massage intervention through literature review,to construct acupoint massage intervention program for multiple myeloma patients with chemotherapyinduced peripheral neuropathy implemented by the primary caregivers.2.Implement the intervention program of acupoint massage for chemotherapyinduced peripheral neuropathy in patients with multiple myeloma, and evaluate the intervention effect of the program.MethodsThis research consists of two parts:1.The construction of an acupoint massage intervention program.Domestic and international databases were searched to grasp the current status of chemotherapyrelated peripheral neuropathy intervention, and the relevant literature was analysed and reviewed to construct the first draft of the intervention protocol.Nine medical and nursing experts in related professions were selected for consultation. On this basis,10 patients were selected for pre-testing,and the intervention protocol was refined and formed into a draft for administration.2.Evaluation of the effectiveness of implementing an acupoint massage intervention program for chemotherapy-induced peripheral neuropathy in patients with multiple myeloma. The convenience sampling method was used to select 68 patients eligible for inclusion from four wards of the department of Hematology of a tertiary hospital in Zhengzhou. The patients were divided into two groups according to the floor of the ward, and the control group and the intervention group were determined by coin flipping. There were 34 patients in each group. The control group was given routine nursing, and the intervention group was given acupoint massage intervention program based on the routine nursing. The chemotherapy-induced peripheral neuropathy Assessment Tool(CIPNAT),quality of Life scale for cancer patients(EORTC QLQ C30),and numerical rating scale(NRS)were used to evaluate the effects of the intervention on chemotherapy-induced peripheral neuropathy symptoms, quality of life, and neuropathic pain in patients with multiple myeloma.IBM SPSS Statistics 20.0software was used to compare the differences between the two groups before intervention, after intervention,1 month after intervention, and 3 months after intervention,as well as the change trend of the study variables over time.Results1.The construction of acupoint massage intervention program.A search of studies on the intervention of massage on chemotherapy-related peripheral neuropathy was conducted from January 2000 to October 2020,and eight articles were finally selected as references to construct the first draft of the acupoint massage intervention. On this basis, two rounds of expert consultation and pre-testing were carried out and the first draft of the intervention protocol was revised,in which the effective recall rate of the two rounds of expert consultation was 100% and the expert authority coefficient was0.85,resulting in the draft of the acupoint massage massage intervention protocol.2.Application of acupoint massage intervention program.A total of 68 patients with multiple myeloma chemotherapy-induced peripheral neuropathy were enrolled.During the study,4 cases were lost, with a loss rate of 5.9%,and an effective sample size of 64 cases;31 cases in the intervention group and 33 cases in the control group.2.1 Peripheral neuropathy: At the end of the intervention,1 month after the end of the intervention and 3 months after the end of the intervention, the sensory dimension, fine activity, general activity dimension scores and total scores were higher for the control patients than for the intervention group, with statistically significant differences(P<0.05).There was no significant difference in the scores of the two groups in terms of exercise dimensions after the intervention, but at 1 month and 3 months after the intervention, the scores of the patients in the control group were higher than those in the intervention group(P<0.05).The generalized estimation equation analysis was performed on the CIPNAT total score and the change trend of each dimension score of the two groups of patients at different time points before and after the intervention. The results showed that the inter-group effect,intra-group effect, and interaction effect of each score of CIPNAT were statistically significant(P<0.05).2.2 Patients’ quality of life: After the intervention,1 month after the intervention, and 3 months after the intervention, the scores of the dimensions of role function, overall quality of life, sleep, and emotional function were statistically significant(P<0.05)between the two groups; There was no statistically significant difference in the scores of fatigue, social function, cognitive function, pain and other dimensions between the two groups(P>0.05),but at 1 month and 3 months after the intervention, the scores of the intervention group in the above four dimensions were better than those of the control group(P<0.05);the scores of physical function and the loss of appetite dimension were not statistically significant between the two groups after the intervention and 1 month after the intervention(P>0.05).The difference in scores at 3months after the end of the intervention was statistically significant between the two groups(P<0.05).There was no statistically significant difference between the two groups in the dimensional scores of dyspnoea, diarrhoea, nausea and vomiting, constipation and economic difficulties at the end of the intervention,1 month after the intervention and 3 months after the intervention(P>0.05).The generalized estimating equation analysis was carried out on the change trend of the scores of the two groups of patients’ quality of life at different time points before and after the intervention. The results showed that emotional function, role function, pain, cognitive function, sleep and overall quality of life, social function of each dimension of the score between groups, within groups and interaction effects were statistically significant(P<0.05).In addition, there were no statistically significant between-group effects(P>0.05)and statistically significant within-group and interaction effects(P<0.05)for the dimensional scores of nausea and vomiting, fatigue, somatic function, and loss of appetite.Diarrhea, constipation, dyspnea, and economic difficulties were not statistically different in the group effect and interaction effect of each dimension(P>0.05),and the effect within the group was statistically significant(P<0.05).2.3 Neuropathic pain: There was a statistically significant difference between the two groups in NRS scores at the end of the intervention,at 1 month after the end of the intervention and at 3 months after the end of the intervention(P<0.05).The trends in pain NRS scores at different time points before and after the intervention in both groups were analysed using generalised estimating equations, and the between-group,withingroup and interaction effects for pain NRS scores were statistically significant(P<0.05).ConclusionThe acupoint massage intervention program led by the main caregiver can improve the symptoms of chemotherapy-induced peripheral neuropathy in patients with multiple myeloma, relieve pain, and improve the quality of life of patients.
