Objective This study set out to explore a possible association between adenoid hypertrophy and nasopharyngeal reflux from stomach contents in children and reveal the importance of gastric reflux.Then the result can improve the anti-acid treatment for adenoid hypertrophy.Methods In this study,we utilized the selected 67 cases who met the inclusion criteria and divided them into 2 groups,the case group included 34 children,and the control group had33 children.Then we investigated the RSI and RFS.During general anesthesia,The nasopharyngeal srcretions of all subjects were collected by a 5ml syringe connected extension tube under nasal endoscopy.We examined the expression of pepsin in nasopharyngeal srcretions which collected from the case group and the control group using PEPTEST and the result was positive when the pepsin concentration was higher than75umol/L.The positive rate of pepsin was calculated respectively and statistically analyzed.Results A total of 30 cases of pepsin were detected in the nasopharynx secretions of all the subjects,the total positive rate was 44.77%.In the case group,pepsin was detect ed in 23 cases and the positive rate was 67.64%(23/34).In the control group,pepsin wa s detected in 7 cases and the positive rate was 21.21%(7/33).The case group had a hi gher positive rate than the control group,the difference was statistically significant(P<0.05).In the case group,15 of 19 patients' nasopharyngeal srcretions with adenoid hypertr ophy who could be defined as grade IV contained pepsin.8 of 15 patients' nasopharyngeal srcretions with adenoid hypertrophy who could be defined as grade ? contained pepsin.In the control group,4 of 12 patients' nasopharyngeal srcretions with adenoid hyp ertrophy who could be defined as grade ? contained pepsin.2 of 13 patients' nasopha ryngeal srcretions with adenoid hypertrophy who could be defined as grade ? contain ed pepsin.1 of 7 patients' nasopharyngeal srcretions with adenoid hypertrophy who could be defined as grade I contained pepsin.In all,the higher the adenoid hypertrophy grade,the higher the expression level of pepsin.According to the data analysis of the 67 cases,In terms of gender and ethnicity,it was not statistically significant(P>0.05).On age(6.23±2.54,7.18±3.14),the difference was not statistically significant(P>0.05),On height(123.± 17.98,128.0 ±19.54),the difference was not statistically significant(P>0.05),In terms of weight,(25.00±8.17,29.55±14.91)the case group was heavier than the control group,is was statistically significant P<0.05).Of all the 67 cases,RSI was greater than 13 points in 2 cases(2.98%),RFS was greater than 7 points in 8 cases(11.94%),And 1 was(1.49%)positive in both scores.Take the positive rate as a gold standard of nasopharyngeal reflux,The sensitivity and specificity of the RSI associated with RFS were 23.3%,94.6%.Conclusion This study identified a novel association between adenoid hypertrophy and nasopharyngeal reflux in children.Nasopharyngeal reflux plays a significant role in the development of adenoid.For children,The sensitivityof the RSI and RFS was low.Therefore,the RSI and RFS were not suit for screening children with adenoid hypertophy whether have the reflux of stomach contents.
