Objective: to investigate the growth characteristics of primary lesions in HAE with pulmonary and brain metastasis using CT and MRI.Data and methods: Imaging data of 76 patients with pulmonary metastasis of hepatic vesicular echinococcosis and 61 patients with brain metastasis of hepatic vesicular echinococcosis were analyzed retrospectively.All patients received plain CT scan and phase iii dynamic enhancement or MRI scan and phase iii dynamic enhancement scan in their abdomen,plain CT scan and enhanced CT scan in their lungs,and plain CT scan and enhanced MRI scan or plain MRI and enhanced CT scan in their head.Combining the imaging data and data of each patient,the characteristics of metastatic lesions and primary lesions were analyzed,such as lesion size,lesion location,number of lesions,vascular invasion and other items.t test and ?2 test were used for comparative analysis in data processing.Results:The maximum diameter of primary hepatic lesion of pulmonary echinococcosis was 25.12 cm,the minimum diameter was 2.90 cm,the mean diameter was 12.38 cm,and the standard deviation was 4.51.38 cases were located in the right lobe of liver,accounting for 50.0%.There were 57 cases with portal vein invasion,accounting for 75.0%.The maximum diameter and minimum diameter of pulmonary lesion in pulmonary hydatid was 11.90 cm and 0.70 cm respectively,and the mean diameter of intrapulmonary lesion was 2.40 cm and the standard deviation was 1.76.Twenty-six cases(34.2%)of the metastatic lesions were distributed in both lungs.The maximum diameter of primary hepatic lesion of brain echinococcosis was 28.71 cm,the minimum diameter was 4.09 cm,the mean diameter was 22.17 cm,and the standard deviation was 3.94.38 cases(62.3%)were located in the right lobe of liver.There were 45 cases with portal vein invasion,accounting for 73.8%.The maximum diameter of the lesion on the head of brain hydatid was 7.15 cm,the minimum diameter was 0.73 cm,the mean diameter of the lesion in the brain was 1.37 cm,and the standard deviation was 0.45.There were 35 cases(57.4%)with bilateral cerebral hemispheres of the metastatic lesions.The primary lesions of alveolar hydatid hydatid and brain hydatid hydatid were at the location of the lesion,and there was no statistical difference in hepatic vein,hepatic inferior vena cava,hepatic artery and hepatic portal vein invasion,number of lesions,etc.(P>0.05).There was a statistical difference in the diameter of primary lesions between the two groups(P<0.05).Conclusion:Primary lesions of pulmonary and brain metastases of echinococcosis vesicular echinococcosis are mainly located in the right lobe of the liver and tend to invade the portal vein,and the metastases tend to diverge in unfixed areas.These growth characteristics provide an important reference for clinical diagnosis and treatment.
