| Purpose To investigate the value of quantitative diffusion weighted MRI for characterizing vertebral fractures as benign or malignant.Materials and Methods 53 vertebral fractures(22 benign vertebral fractures and 31malignant vertebral fractures)were enrolled.At 1.5T,every vertebral fracture was imaged by Conventional MRI and DWI(b-values 50,200,400,600,800 s/mm2).Differences between SI,R,ADC values in benign and malignant vertebral fractures were assessed quantitatively.Receiver operating characteristic analysis was performed to evaluate MRI diagnostic performance.Results ADC50-400(the ADC calculated with b=50 and 400 s/mm2)showed the highest AUC(0.956).ADC50-400≤1.337×10-3mm2/s,was determined for dfifferentiating malignant vertebral fractures from benign vertebral fractures(sensitivity 96.8%;specificity 86.4%).There were higher ADC50-400s(1.928×10-3mm2/s vs.0.832×10-3mm2/s,p<0.05)in benign vertebral fractures compared with those in malignant vertebral fractures.SIA800 and R800 showed high AUC(0.894 and 0.858).There were lower SIs、R s in benign vertebral fractures compared with those in malignant vertebral fractures.Conclusion Quantitative diffusion weighted MRI is useful for differentiation of benign and malignant vertebral fractures.ADC50-400s offer to distinguish malignant fractures from benign fractures with the highest accuracy.SIA800 is helpful to distinguish malignant fractures from benign fractures. |
