As the most basic human right that reflects social equity,health is the responsibility of the government,for good health of people is a manifestation of moderate prosperity of society.Human resource is the first resource,which is the most important resource for the improvement in medical health.Good health is an important symbol of a prosperous nation and a strong state,so no good health,no moderate prosperity.As the most important part of primary medical health,rural medical health is undoubtedly the top priority of medical health reform and development in China.In this critical period when the whole province follows the requirement of the State Council that "basic medical health should be guaranteed,primary medical health should be strengthened and the mechanism should be established" to promote the reform of the medical health system,implement the health-oriented poverty alleviation project and establish a high-quality and efficient medical health service system so as to accelerate the construction of primary health talent team,give play to the role of medical staff and improve the capacity and level of primary medical health services,this research analyzes the status quo of health human resources in Longling County and provides relevant measures and recommendations.Objective:This research aims to understand and master the status quo and existing problems of health human resources in Longling County,explore the causes and influencing factors of the problems,and combine with the actual situation of health human resources in Longling County to analyze in depth the introduction,training,motivating and guarantee of health talent team with a view to proposing measures and recommendations for the development and utilization of health human resources in Longling County and providing a reference for other similar regions to improve the development and utilization of primary health human resources.Methodology:The qualitative method and quantitative method were combined for descriptive research on the status quo.The multi-stage sampling method integrating stratified and cluster random sampling was used to for questionnaire survey,group discussion and in-depth interview among respondents at the levels of county,town and village,including their basic information,education background,professional level,job title,salary,training attendance,training effects,and satisfaction with training.Data entry with Epidata3.0 and,SAS9.4 is used to achieve the statistical analysis,a=0.05.Results and conclusions:First,the health workers in Longling County have problems of shortage,low quality and unstable team.According to the data of Longling County Health Bureau,there are only 3.3 health technicians available for every 1,000 people,lower than the national level and provincial level of Yunnan,and the distribution of age,professional title and education background is inferior to the national average.It is difficult to recruit and retain talents,who show a reverse flow.Second,health workers have low income satisfaction,and village doctors urgently need social security.The annual income of county-level health workers was 60,000(50,000,70,000)yuan in 2017,with a satisfaction rate of 60.8%,that of town-level health workers was 60,000(50,000,72,000)yuan in 2017,with a satisfaction rate of 69.6%,and that of village-level health workers was 30,000(20,000,43,000)yuan,with a satisfaction rate of only 25%,which may be because the problem of pension of village doctors needs to be solved.Third,the public health service capacity at the levels of town and village has been improved,while the basic medical service capacity still needs strengthening.In recent years,the public health service capacity has been developed in Longling County,but the medical service capacity still needs strengthening,for it cannot meet the daily needs of residents due to the shortage and low quality of health workers as well as the difficulty in retaining talents.The increase in public health services costs a lot of energy of health workers and thus affects the medical services,which have shifted from more treatment and less prevention to more prevention and less treatment.The shortage of doctors and medicine in the health institutions at the levels of town and village impedes the improvement of medical service capacity.Fourth,the training channels,contents and modes of health workers need to be adjusted to improve the training effects.The training for health workers in Longling County is mainly organized by the government and superior medical institutions,which have no opportunities to participate in the training organized by NGOs.Theoretical knowledge training is the general training content for health workers at the levels of county and town,and the training of medical practice is less provided.It is mainly short-term training with unobvious effects,and despite the advantages of remote training,its content is lagging and its effects are not obvious.More than 60%(64.0%)of health workers at the levels of county,town and village believe that the existing training modes have unobvious effects.Fifth,the policy and system need to be improved to fit the reality of primary health.Due to the backward economy and society in Longling County,some policies and systems are not consistent with the reality,for example the existing authorized strength hinders the development of primary health human resources and the targeted-area student policy cannot immediately meet the urgent human resource needs of medical institutions.The recruitment policy adopted by local governments that requires a bachelor’s degree or deputy senior title improves the level of health human resources to a certain extent,but it still cannot fully satisfy the actual situation.Measures and recommendations:First,it should be the premise to strengthen top-level design so as to reinforce the dominant functions of the government.Second,it should be the objective to meet the demands so as to increase the quantity and optimize the structure.Third,multi-channel training should be taken as the means to improve the competency of health workers.Fourth,it should be a breakthrough to improve remuneration so as to mobilize the enthusiasm of health workers.Fifth,it should be the goal to promote service capacity so as to improve knowledge,ability and quality of health workers.
