BackgroundHypertension is a chronic disease,the course of the disease has a hidden,with the extension of the course of disease,can be associated with systemic diseases such as heart,brain,blood vessels,kidney and so on,It can lead to functional or organic changes in these organs.It is not obvious and is easy to be ignored that the early clinical symptoms of hypertension.When these symptoms appear,the treatment is very difficult.Therefore,it is particularly important that early diagnosis and treatment of hypertension.In patients with early hypertension,although the structure of the heart is normal,due to the increased of peripheral resistance and the decreased of left ventricular compliance,the early effects of cardiac function is mainly reflected in the left ventricular diastolic dysfunction.In recent years,heart failure caused by left ventricular diastolic dysfunction is increasing year by year.It is the most common that diastolic heart failure caused by hypertension.Therefore,the early diagnosis of left ventricular diastolic dysfunction is closely related to the prevention and treatment of heart failure.Renal dysfunction is a risk factor for arrhythmia,heart failure and sudden cardiac death.Hypertension is the main cause of renal insufficiency.The evaluation indexes of abnormal renal function include urea nitrogen,creatinine,and beta2-microglobulin.Among them,beta2-microglobulin is a sensitive index of early renal function damage,and is better than creatinine,urea nitrogen and other indicators.At present,it has been found that beta2-microglobulin is a new marker of cardiovascular events and death,and it may increase the possibility of ventricular remodeling.A large number of experimentals have studied the relati-onship between beta2-microglobulin and hypertension nephropathy,but the study of the relati-onship between hypertension and heart disease has not been reported.The aim of this study was to investigate the relationship between beta2-microglobulin and left ventricular structure and function in patients with early hypertension.ObjectiveThe present study was conducted to determine the serum levels of beta2-microglobulin and the left ventricular structure and function in patients with hypertension and non hyperten-sion,to explore the relationship between beta2-microglobulin and left ventricular structure and function,and to further guide the prevention of ventricular structure and function in the early stage of hypertension.MethodsIn this study,we selected 120 cases who were initially diagnosed with elevated blood pressure(56 patients were male,64 patients were female),with an average age of 55 years,and 120 cases of healthy individuals at the same time in our hospital physical examination center(42 patients were male,78 patients were female),with an average age of 53 years.All patients were given general information(such as gender,age,diabetes,kidney disease,et al.),and were collected blood samples for laboratory tests of liver function and renal function,blood sugar and so on.All subjects were examined for the determination of graph of left atrial diastolic maximum diameter,left ventricular diastolic diameter,left ventricular wall thickness,left ventricular ejection fraction,E peak,A peak,Ea peak,Aa peak index by echocardiography.SPSS20.0 software was used for statistical analysis of all data,and the difference with P < 0.05 was statistically significant results.The level of serum beta2-microglobulin was significantly different between hypertension group(1.42±0.15)and non hypertension group(1.37±0.11).There was no significant difference in E/A value between hypertension group(11.62±1.82)and non hypertension group(11.11±1.16),but there was significant difference in E/Ea value between the two groups.It is indicated that E/Ea can better reflect left ventricular diastolic dysfunction than E/A in early hypertension.In addition,there was a significant positive correlation betwe-en E/Ea and the beta2-microglobulin in hypertension group group(The correlation coefficients r was 0.471,P<0.0010).After multivariate logistic regression analysis,the data showed that the independent risk factors of left ventricular diastolic dysfunction were age,systolic blood pressure,left ventricular wall thickness,left atrial diameter,beta2-microglobulinin for the patients with hypertension among them,beta2-microglobulin: OR=1.570,95% CT: 1.1005-2.147,P=0.013.ConclusionsCompared with healthy people,the serum beta2-microglobulin level and left ventricular diastolic function of patients with early hypertension were significantly different,and there was a significant positive correlation between them.And beta2-microglobulin is an indepen-dent risk factor of left ventricular diastolic dysfunction in patients with hypertension.
