Clinical Observation Of Zishen Yi Jing Huoxue Decoction Combined With Climen In The Treatment Of Kidney Deficiency And Blood Stasis Type DOR

Objective:To observe the clinical effect of Zishen Yijing Huoxue decoction combined with Climen in treating diminished ovarian reserve of kidney deficiency and blood stasis type,to explore the possible mechanism of action,and to provide theoretical basis for clinical popularization and application.Methods:Selected 66 patients who met the diagnostic criteria for diminished ovarian reserve of kidney deficiency and blood stasis type in the infertility department outpatient clinic of The hospital of shanxi university of chinese medicine from january 2019 to october 2019.According to the random number table method,they were divided into treatment group 33 cases and control group 33 cases.The treatment group was treated with Zishen Yijing Huoxue decoction combined with Climen,and the control group was treated with Climen for 21 days each cycle,with a total of three cycles.The SPSS 25.0 statistical software was used for statistical analysis by observing the improvement of serum sex hormone levels?FSH?LH?FSH/LH?E₂?,Anti-Mullerian hormone?AMH?,Antral follicle count?AFC?,and clinical symptoms and concurrent symptoms before and after treatment.Results:?1?The experiment included 63 cases,the treatment group fell off 1 case,the control group fell off 2 cases.Age and course of the two groups:There was no significant difference between the two groups before treatment?P>0.05?.From the age distribution of the two groups,we can see that the incidence of DOR is positively correlated with age.?2?Comparison of total effective rate and TCM syndrome efficacy:After treatment,the total effective rate of the treatment group reached 87.5%,which was higher than that of the control group 77.42%.After rank sum test,the difference between the two groups was statistically significant?P<0.05?,suggesting that the clinical efficacy of the treatment group was more significant than that of the control group.For the curative effect of TCM syndromes,the total effective rate of the treatment group was 90.63%,which was higher than that of the control group 74.19%.After rank sum test,the difference between the groups was statistically significant?P<0.05?,indicating that the curative effect of TCM syndromes in the treatment group was more significant than that in the control group.?3?Comparison of total integral of TCM symptoms:After treatment,the total integral of TCM symptoms in both groups was significantly lower than before,and the difference was statistically significant?P<0.01?;the improvement degree of the treatment group was significant,and the difference between the two groups was statistically significant?P<0.05?.?4?Comparison of TCM single symptom score:After treatment,the scores of the two groups in regulating menstruation and other secondary symptoms were lower than before,and the difference was statistically significant?P<0.05?.After treatment,the improvement degree of the treatment group was more obvious than that of the control group in the aspects of menstrual volume,menstrual period,menstrual cycle,insomnia,and abdominal pain after treatment,and the difference between the groups was statistically significant?P<0.05?.For transchromatic meridian,dizziness tinnitus,lumbosacral soreness,decreased libido,pale complexion or dark spots,the scores of both groups were lower than before,but there was no significant difference between the two groups?P>0.05?.?5?Serum hormone levels and ultrasound indexes were compared between the two groups:After treatment,the FSH?LH?FSH/LH?E₂ levels of the two groups were lower than before,and the AMH?AFC levels were higher than before,and the difference was statistically significant?P<0.01?.The difference between the two groups was statistically significant?P<0.05?.The FSH/LH of the treatment group and the control group decreased slightly,but the difference between the two groups was not statistically significant?P>0.05?.?6?Safety observation:There were no adverse reactions in both groups.Conclusion:?1?Zishenyi Meridian Huoxue decoction combined with Klingmeng can effectively reduce the levels of follicle-stimulating hormone?FSH??luteinizing hormone?LH??FSH/LH and estradiol?E₂?on the third day of menstruation,increase the level of anti-mullerian hormone?AMH??antral follicle count?AFC?,which can improve ovarian function in patients with diminished ovarian reserve.?2?Both Zishen Yijing Huoxue decoction and Climen were superior to Climen in the improvement of TCM symptom score?menstrual volume,menstrual period,cycle,insomnia and dream,abdominal pain?,serum FSH?LH?E₂?AMH value and sinus follicle number.?3?Zishen Yijing Huoxue decoction combined with Climen in the treatment of kidney deficiency and blood stasis type diminished ovarian reserve is safe and effective,can be popularized in clinical application.
