Objective(1)To explore the characteristics of gut microbiome in colorectal cancer patients with different Traditional Chinese Medical Syndrome and find out the difference.(2)To analyze the typical gut microbiome in colorectal cancer patients with different syndromes and the correlation between the gut microbiome in spleen insufficiency with damp retention and immune-related factors concretely.(3)To explore whether the tumor-associated macrophage who can change the tumor immune microenvironment is the bridge of Tradition al Chinese Medical Syndrome and the microbiome in spleen insufficiency with damp retention or not.Methods In this study,we judged the Traditional Chinese Medical Syndrome of 32 enrolled patients with first-episode colorectal cancer according to the syndrome scale with the basic information recorded and the samples collected.Fecal DNA was extracted for amplified sequencing,and the sequencing results were processed by cluster analysis.Then we analyze the complexity of samples and the differences of the groups.Il-6,il-8 and il-10 were detected by Elisa.Analyzed the correlation between gut microbiome and the Elisa results.Compared the expression of the tumor-associated macrophage(TAMs)in th e enrolled patient,and analyzed the correlation between the microbiome in spleen insufficiency with damp retention and TAMs.Results According to the Alpha diversity analysis,we found the gut microbiome in the qi stagnation and blood stasis retention is the abundant with the spleen insufficiency damp retention behind.The analysis of inter-group differences showed that the gut microbiome in different syndromes varied,and the qi stagnation and blood stasis retention was the most different.The gut microbiome of the spleen insufficiency damp retention mainly included Ruminococcus and Catenibacterium with other representative microbiome such as Stercorea,Peptococcaceae,Paraprevorella.The expression of il-6 and il-10 in patients of the spleen insufficiency damp retention were significantly higher than other syndromes.Heat map analysis showed that the gut microbiome distribution of patients with higher il-6 and il-10 was similar to that of patients of the spleen insufficiency damp retention,whose proportion of Ruminococcus was relatively high.The infiltration of CD206+in the slices of patients of the spleen insufficiency damp retention was significantly more than others.Conclusion The distribution of the gut microbiome in colorectal patients was correlated with the Traditional Chinese Medical Syndrome.The main gut microbiome in patients of the spleen insufficiency damp retention were Ruminococcus and Catenibacterium.The gut microbiome might active the macrophages polarized to the M2 directly or indirectly and let TAMs interact with cancer cells,which stimulated the increase of il-6 and il-10 in serum,resulting in the spleen insufficiency with damp retention.
