Bjective:To systematically evaluate the changes of brain iron deposition,brain structure and brain function in children with beta-thalassemia by cross-sectional study of children with beta-thalassemia by MRI.Materials and Methods:Thirty-four children(22 males and 12 females,aged 9-14 years,average 11.80+1.97 years)with severe Beta-type Mediter-ranean were enrolled in the First Affiliated Hospital of Guangxi Medical University from January 2017 to December 2018.Twenty-nine normal children volunteers(15 males,14 females,age 9-14 years,average 11.10+1.50 years)were recruited from January 2017 to December 2018.The liver gradient was measured by T2WI-flair with MRI.Multi-echo T2WI,brain spin multi-echo T2WI,SWI,3DT1WI,BOLD sequence scanning.Hepatic gradient multi-echo T2WI and SWI sequences were used to evaluate intrahepatic iron deposition and choroid plexus iron deposition in children with beta-thalassemia severely;brain spin multi-echo T2WI sequences were used to quantitatively measure basal ganglia iron deposition in children with beta-severe thalassemia;voxel-based morphological method(VBM)and surface-based morphological analysis(SMA)were used to evaluate the volume of gray matter and subcutaneous volume in children with beta-severe thalassemia.Change;Independent Component Analysis(ICA)was used to assess the change of default network function in children with severe severe ground poverty.Result:1.There was no correlation between liver T2*value,choroid plexus iron deposition,bilateral caudate nucleus head,globus pallidus,putamen,thalamus,substantia nigra and red nucleus iron T2 mapping value in the poor group.2.Iron deposits in bilateral caudate nucleus head,globus pallidus,putamen,thalamus,substantia nigra and red nucleus in severe beta group were higher than those in normal group(P<0.05).3.The iron deposits in bilateral caudate nucleus head,globus pallidus,putamen,thalamus,substantia nigra and red nucleus in severe beta group were higher than those in normal group(P<0.05).4.VBM results based on P<0.05 non-correction and P<0.05 cluster FDR correction:the volume reduction structure of gray matter and brain areas in the poor group were bilateral talus cortex,bilateral cuneiform cortex,bilateral lingual gyrus,bilateral basal ganglia(caudate nucleus,pallidum,amygdala nucleus,nucleocapsid),brainstem and cerebellum;the volume enlargement structure of gray matter and brain areas in the poor group were larger than those in the normal group,including right insular lobe and right central part.The islands cover,bilateral posterior central gyrus,left anterior central gyrus,left parietal lobule,left superior temporal lobule and right inferior temporal gyrus.5.Subcutaneous volume measurements showed that the volume of bilateral caudate nucleus head,putamen,amygdala,cerebellum,right globus pallidus and right hippocampus in the poor group was significantly smaller than that in the normal group(P<0.05).6.Independent Component Analysis(IAC)Results:Compared with the normal group,the functional enhancement areas in the poor group included left middle temporal gyrus,left inferior orbital frontal gyrus,left superior temporal gyrus,right middle temporal gyrus,left posterior central gyrus,left medial and lateral cingulate gyrus,left inferior parietal gyrus,left superior parietal gyrus,right inferior parietal gyrus,right superior frontal gyrus and right anterior central gyrus.The brain areas with impaired function in the thalassemia group were right middle temporal gyrus,left rectus gyri,right anterior cuneiform lobe,right inferior triangular frontal gyrus,left middle frontal gyrus,right superior frontal gyrus and left supplementary motor areas.Conclusion:1.There was no correlation between liver T2*value,choroid plexus iron deposition,bilateral caudate nucleus head,globus pallidus,putamen,thalamus,substantia nigra and red nucleus iron T2 mapping value.2.The iron deposits in bilateral caudate nucleus head,globus pallidus,putamen,thalamus,substantia nigra and red nucleus in severe beta group were higher than those in normal group(P<0.05).3.The iron content of bilateral caudate nucleus head in beta severe thalassemia group and normal group had significant difference(P<0.05).
