| Objective: Compare the recurrence of different pathological types of lumbar disc herniation after 2 years of spinal endoscope treatment,and observe whether there are differences between the three,and guide the clinical treatment of lumbar disc herniation.Methods: Based on a retrospective study approach,we collected data from194 lumbar disc herniation patients through a retrospective analysis of each patient’s medical records.The patients were divided into three groups according to the different pathological conditions observed during imaging and operation: protruding group,prolapse group and free group.Compare the general conditions of patients in each group,adjust the risk factors for postoperative recurrence,analyze the relationship between recurrence and pathological type in two years after operation by COX multivariate regression.Results: There were 194 patients treated by spinal endoscopy,81 in A group,67 in B group and 46 in C group.All patients completed surgery without complications such as nerve root injury,rupture of the dural sac,and intervertebral space infection.126 males and 50 females,aged between 32 and 62 years,had a total mean age(43.55 ± 5.01)years.There was no significant difference in age,sex and course of disease among the three groups(P>0.05),and there was no significant difference in VAS score and JOA score between the three groups(P>0.05).There were 18 patients with postoperative recurrence,A group 4,B group 8,C group 6,postoperative recurrence time 9.89±3.71 months(5 to 18 months),postoperative 6 months recurrence 3,postoperative 6 to 12 months recurrence 10,postoperative 12 to24 months recurrence 5,of which 13(72.2%)were within 1 year after operation.Compared with the recurrence rate of different time after operation in the three groups,there was no significant difference(P>0.05).After analyzing the risk factors affecting recurrence and adjusting other risk factors,there was no significant difference in the risk of recurrence among the three groups(P>0.05).Conclusion: 1.The treatment of lumbar disc herniation with different pathological types by spinal endoscope has remarkable curative effect and no obvious difference in postoperative recurrence.2.Postoperative recurrence often occurs within half a year to one year after operation,patients should pay special attention to postoperative waist protection and avoid heavy physical work,patients should wear waist support protection. |
