Objective:To explore the correlation of amino acids and acyl carnitine with intestinal polyps in T2 DM patients,so as to provide relevant basis for early prevention and treatment of type 2 diabetes mellitus with intestinal polyps.Methods:499 patients with type 2 diabetes who were hospitalized in the Department of Endocrinology 1 of Dalian Medical Affiliated Second Hospital from January 1,2018 to January 1,2020(in line with the diagnostic criteria set by the WHO Diabetes Committee in 1999)were selected.According to the results of colonoscopy, they were divided into two groups: 200 patients in the group with intestinal polyps,115 males and 85 females, with an average age of(60.26 ± 9.40)years.There were 299 cases in the group without intestinal polyps,147 males and 152 females, with an average age of(58.90 ±13.83)years.The basic data collected were gender, age, height, weight, body mass index(BMI),course of type 2 diabetes.Fasting blood glucose(FPG),fasting insulin(fins),glycosylated hemoglobin(Hb A1c)and insulin-related index(HOMA IR)were collected from venous blood 8 hours after fasting.Amino acids and acyl carnitine were detected by means of metabonomics such as chromatography-mass spectrometry(MS)and nuclear magnetic resonance(NMR).To compare the level of basic and clinical data between the two groups,to analyze the correlation between the above indicators and type 2 diabetes patients with intestinal polyps, and to analyze the related factors of T2 DM with intestinal polyps. Results: In the basic data of the two groups, there was no significant difference in age, gender and weight(P > 0.05),but height and body mass index(BMI)were statistically significant(P < 0.05).In clinical data, the course of type 2 diabetes,HOMA IR,Hb A1 c,leucine,homocysteine,alanine,tyrosine,glutamate,tryptophan,phenylalanine,C14,c14-oh,C20,C24 and C26 in the group with intestinal polyps were significantly higher than those in the group without intestinal polyps(P < 0.05).Carry out logistic for the above statistically significant indicators After analysis, the course of type 2 diabetes, body mass index(BMI),glycosylated hemoglobin, leucine, homocysteine, alanine, tryptophan, glutamic acid,C20 were significantly correlated with the occurrence of intestinal polyps in T2 DM patients(P < 0.05).The or value of course,BMI,Hb A1 c,leucine, tryptophan, glutamic acid and alanine of type 2 diabetes decreased after the factors of HOMA IR were added. Conclusion: The incidence of intestinal polyps in patients with type 2 diabetes is positively correlated with the course of diabetes, body mass index(BMI),HOMA IR,Hb A1 c,leucine, homocysteine, alanine, tryptophan, glutamic acid and C20.The course of type 2 diabetes, body mass index, glycosylated hemoglobin, leucine, tryptophan, glutamic acid and alanine were related to insulin resistance.
