| Objective:To compare the difference of plasma Orexin-A(OXA)level between male patients who suffered the alcohol withdrawal syndrome(AWS)and the group of healthy adult males. And analyze the correlation between the change of OXA concentration and the severity of withdrawal symptom and craving in patientsMethods:From July 2017 to August 2019,60 admission patients with alcohol dependence who met the experimental enrollment criteria were enrolled as the research group, and 60 healthy volunteers who met the inclusion criteria were collected as the control group.Screening the health control subjects by the scale of Michigan alcoholism screening teat and collecting the general demographic statistics dates in the two group and the clinical situation of alcohol consumption before recruited. The plasma concentration of OXA,total cholesterol(CHOD),alanine transaminase(ALT),aspartate aminotransferase(AST),creatine kinase(CK),blood sugar and potassium should be tested in the research groups on the enrollment day(baseline)and day 14.However,this above indicators were tested only once at the baseline in the control group. The Clinic Institute Alcohol Withdrawal Syndrome Scale(CIWA-Ar)and visual analogus scale(VAS)were Used to evaluate the severity of alcohol withdrawal symptoms as well as craving in the research group.SPSS22.0 statistical software was used to research on the difference of study group and control group for verifying the association of OXA between alcohol withdrawal symptom and cravingResults:1.The plasma Orexin-A(OXA)concentration of study group(302.50±46.77pg/mL)were higher than the control group(54.42±8.60pg/mL)on the first day of enrollment, and the difference was statistically significant(t=40.41,P=0.00).Meanwhile,there were statistically significant differences in the concentration of CHOD,AST,ALT,CK,blood glucose and potassium between study group and control group(all P<0.05)2.Compare the level of OXA in patients with mild withdrawal response(281.96±39.49pg/mL)with the moderate or severe subjects(318.51 ±40.70pg/mL,315.60±53.95pg/mL),there were significantly statistic differences(F=4.67,P=0.01).But there was no statistically significant difference in plasma OXA levels between moderate and severe withdrawal subjects(all P>0.05)3.Comparing the plasma OXA concentration in patients before and after 2 weeks treatment,it has been found that the plasma OXA concentration in patients before treatment(314.73±41.80pg/mL)was significantly higher than the after(282.29±49.69pg/mL),and the difference has statistical significant(t=3.42,P=0.00)4.There was a positive correlation between plasma OXA concentration and the severity of withdrawal response, and craving at the baseline in the study group(r=0.34,P=0.00;r=0.47,P=0.00)5.The level of OXA was positively correlated with the severity of anxiety and agitation during 2 weeks treatment(r=0.29,P=0.01;R=0.22,P=0.03)6.Multiple liner regression analysis showed that Plasma level of OXA and CK had impact on alcohol the withdrawal severity(P=0.01,P=0.01),and OXA level also had impact on the craving at baseline(P=0.00)。Conclusion:Plasma OXA concentrations in male patients were higher than the healthy control groups at acute alcohol withdrawal period. There was a positive correlation between the severity of withdrawal response and psychological craving with acute alcohol withdrawal and plasma OXA level in male patients. The patients who with high level of OXA are prefer to suffer more serious withdrawal response and psychological craving. |
