Objective:To explore clinical biological characteristics,diagnosis and rational surgical treatment of thyroid papillary microcarcinoma.Method:Retrospectively analyze 102 cases with papillary thyroid carcinoma,who accepted surgical treatment in affiliated hospital of Taishan Medical University between November2012 and February 2016,and all patients accepted ultrasound examination before operation.Make statistics of preoperative ultrasonic results and clinical features according to the results of the postoperative pathology.Surgery can be divided into two groups: near/total thyroidectomy and lobectomy.We discuss the prognosis of patients of different operative methods according to the incidence of postoperative complications and historical literature.Results:The ratio for man to women was 1:2.47.The age of these patients was between24~80 years old,and the median age was 49.5 years old.79 cases(77.94%)of preoperative ultrasound examination showed signs of malignant nodules.71 cases were microcarcinoma patients according to the postoperative pathology results.There were 87 patients underwent lymph node dissection,43 cases of which have lymph node metastasis.Statistical analysis showed that the lymph node metastasis rate between multiple foci and single tumor are obviously different.There were 16 cases of parathyroid injury and hypocalcemia after surgery,5 cases of unilateral recurrent laryngeal nerve injury,3 cases of subcutaneous hematoma,1 case of lymph leakage.The incidence of complications of lobectomy group was 15%,and near/total thyroidectomy group was 26.67%,higher than the lobectomy group,but χ2 test results showed it has no obvious statistical significance.Conclusion:1.In recent years,PTMC accounted for a higher proportion in newly discovered papillary thyroid carcinoma patients;2.Ultrasound is the preferred examination method;3.PTMC patients need positive surgical treatment instead of being followed up under the special circumstances in our country;4.Choosing reasonable surgical treatment for patients according to the characteristics of thyroid nodules,concomitant diseases,operators’ professional technical level,etc.,and weighing the incidence of complications and risk of recurrence,to make patients obtain the maximum benefit.
