ObjectiveStudy on coagulation indexes of patients with esophageal cancer,the relationship between the comprehensive analysis of patients with esophageal cancer and its clinical characteristics of coagulation function,find the factors related to esophageal cancer patients with high blood coagulation status,provide reference basis for prevention and treatment of hypercoagulable status of patients with esophageal carcinoma.MethodsPatients with esophageal cancer who were treated in Affiliated Hospital of Tai’an Medical College from March 1,2015 to October 31,2016 were selected according to the screening criteria.After screening,a total of 64,marking the general clinical data including name,gender,age,hospital number,pathological type,stage,there is no underlying diseases,there is no PICC catheter,physical condition and imaging data(upper gastrointestinal barium,CT,magnetic resonance imaging)and treatment methods;and records of patients blood coagulation index: plasma prothrombin time(PT),activated partial thromboplastin time(APTT),thrombin time(TT),fibrinogen(Fi B),two D-dimer(D-D),platelet(PLT).According to the basic principle of the same period,39 healthy subjects were randomly selected,and the clinical data and blood coagulation indexes were recorded.SPSS 17 for Windows statistical software was used to organize and analyze the data,the sample size of the control group was compared with the U test,the difference was statistically significant P<0.05,P<0.01 difference was statistically significant.Results1.Analysis of general data of experimental group and control group of esophageal carcinoma.The healthy control group included 39 people,including a total of 21 male and female,aged 37-78 years old,with a median age of 55 years(range).There were 50 males and females in the experimental group of,with an age range of 41-80 years and a median age of 64 years(range,2).Two groups of patients in the general demographic characteristics of the match,the correlation can be compared.2.Comparison of abnormal number and abnormal rate of esophageal cancer group and control group.There were 64 cases of esophageal cancer and abnormal coagulation,the abnormal rate was 67.2% in the control group(n = 39)and abnormal coagulation in the treatment group(n = 6).The abnormal rate was 15.3%.The difference between the two groups was statistically significant(P<0.01).3.Comparative analysis of coagulation indexes between experimental group and control group.Esophageal cancer group in plasma FIB,D-D and platelet count was significantly increased,the difference was statistically significant(P<0.01);the content changes of esophageal cancer group compared with the control group APTT,PT and TT are not obvious,the difference was not statistically significant(P>0.05).4.Characteristics of coagulation test in esophageal cancer experimental group.In 64 cases of esophageal cancer patients,43 cases(67.2%)with different degree of abnormal coagulation results;the increase of D-D number,there were 36 cases(56.3%),followed by FIB increased in 21 cases(32.8%),again platelet increased in 18 cases(28.1%),two coagulation test results are more than 20 cases of reference range(31.25%),FIB,D-D value of two common number of patients increased most,there were 16 cases(25%);blood coagulation analysis with three indexes of 10 cases of common beyond the normal reference range(15.63%),FIB,D-D,PLT three elevated the largest number of patients in 6 cases(9.38%).5.Relationship between coagulation test results and clinical features in esophageal carcinoma.5.1.Characteristics of coagulation indexes in patients with esophageal cancer at different ages.Esophageal cancer patients of different age group,FIB and D-D,increased at the age of 60 patients with less than 60 years group content in varying degrees,the difference was statistically significant(P<0.05);the content changes of coagulation indexes of PT,TT,APTT and PLT is not obvious,the difference was not statistically significant(P>0.05).5.2.Comparison of various coagulation indexes in different TNM stages(stage Ⅰ,stage Ⅱ and stage Ⅲ a nd Ⅳ).According to the 2009 UICC seventh edition of TNM staging of esophageal cancer,64 cases of esophageal cancer were selected,I and II patients with a total of 26 cases of non advanced group,stage III and IV patients for a total of 38 cases of late stage group.Advanced esophageal cancer patients compared with non advanced patients,plasma Fib,D-D and PLT content on the advanced group of patients was significantly increased,the difference was statistically significant(P<0.05);and in the late PT,TT,APTT content in plasma of group and non group than in the late did not change significantly,the difference was not statistically significant(P>0.05).5.3.Comparison of blood coagulation indexes of different pathological types.In patients with different pathological types of esophageal cancer,the changes of coagulation index and PLT in patients with squamous cell carcinoma and adenocarcinoma group were not significant,and the difference was not statistically significant(P>0.05).5.4.Characteristics of coagulation indexes in patients with different gender.There was no significant difference between the male group and the female group in the changes of blood coagulation indexes and PLT levels in different gender patients with esophageal cancer(P>0.05).5.5.Characteristics of coagulation indexes in patients with different ECOG scores.Different physical condition of patients with esophageal cancer,poor physical condition group(ECOG > 2)PLT level than physical condition group increased,the difference was statistically significant(P<0.05);the content of plasma FIB,D-D,and physical condition group increased significantly,there was significant difference in statistical significance(P<0.01);the plasma levels of PT,TT,APTT content,and the physical condition group did not change significantly,the difference was not statistically significant(P>0.05).5.6.Characteristics of patients with disease and coagulation index.There was no significant difference in the changes of coagulation index and PLT in patients with esophageal cancer complicated with disease and without disease(P>0.05).5.7.The characteristics of coagulation indexes in the treatment group and the treatment group.Comprehensive treatment of plasma D-D increased significantly,compared with the untreated group,the difference was statistically significant(P<0.05);the content of comprehensive treatment of plasma PT,TT,APTT,FIB and PLT and compared with the initial treatment group did not change significantly,the difference was not statistically significant(P>0.05).5.8.Characteristics of coagulation indexes in patients with or without PICC catheter.Esophageal cancer patients with PICC catheter for increasing the content of D-D,and no PICC tube were compared,the difference was statistically significant(P<0.05);and the content changes of PT,TT,APTT,Fib,FIB and PLT are not obvious,the difference was not statistically significant(P > 0.05).Conclusion1.In patients with esophageal cancer,more than 67% of patients have an uneven degree of coagulation function change,which is characterized by hypercoagulability.2.Laboratory blood tests were most common with fibrinogen,D-D,and platelets exceeding normal levels.3.The change of coagulation function in patients with esophageal cancer is related to the age,TNM staging,ECOG score,PICC catheterization and comprehensive treatment.
