Research On Compensation Mechanism Of Public Hospital At County Level In Jilin Province From The Perspective Of Drug Zero Rate

Objective: The reformation of compensation mechanism in public hospitals has always been the priority among priorities in the medical-sanitary system reform in China and it is the key to ensure the public good of public medical and health.After the market-oriented reform in medical and health area,the public hospitals compensate for the cost through the way of drug-based care because of the insufficient government subsidies,low pricing of medical services and other factors which aggravates the induced excessive medical behaviors and infringes the public welfare of public hospitals.It also brings a series of social problems.The zero margin drug profit aims to solve the above problems.However,adjusting the price of medical services and increasing government subsidies cannot completely fill the gap caused by the cancellation of the mark-up of drug.After the implementation of zero margin drug profit,the hospital's income declined sharply.From the sight of patients,"expensive medical treatment" still exists at the same time.On the premise of canceling the mark-up of the drug,this paper studies and discusses the compensation mechanism of public hospitals.It solves the problem of "expensive medical treatment and drug cost" by establishing a reasonable compensation subject,compensation method and compensation level to make the hospitals work effectively and plays the role of safeguarding the health for the masses.Through the research on compensation method of county-level public hospitals,this study finds out the problems existing in the compensation mechanism and puts forward more scientific and reasonable suggestions to improve the compensation method.Methods: This paper starts from the definition of pharmacoeconomics and build a research frame about the compensation method of county-level public hospitals.This paper bases on the theory of supply-induced demand and doctor-patient agency.It uses the literature analysis method to make a sort-out about situation of compensation mechanism in public hospitals at home and abroad and the historical evolution of compensation mechanism in public hospitals in China.It collects the relevant data about 80 county-level public hospitals of 9 cities(states)in Jilin Province and uses the descriptive statistical method to analyze the main data such as the proportion of the medicine,the income from drugs,medical fee of each person and so on.Conclusion: In order to solve the problem of "expensive medical treatment",we should make a scientific and reasonable medical service price,reform the compensation method of county-level public hospitals and increase the investment to county-level public hospitals gradually.Except promoting the zero margin drug profit,we should pay more attention to the government's financial investment and hospital‘s related compensation methods because this is the key to ensure the public good of public hospitals and the key to measure the medicalsanitary system success or not.This paper puts forward five suggestions on the compensation method to county-level public hospitals.It provides some constructive suggestions for the government to improve the comprehensive reform and policy of medical prices in countylevel public hospitals.
