Objective:Urolithiasis is one of the most common urological diseases at present,and the recurrence rate is high.At present,the treatment of urolithiasis at home and abroad is mainly to deal with calculi,and the causes of urolithiasis are still in the exploration stage.The purpose of this study was to explore the correlation between urolithiasis in adult urinary system and traditionl Chinese medicine(TCM)constitution,and to provide a new direction and strategy for the prevention and treatment of urolithiasis.Methods:A total of 998 adult patients with urinary calculis in our center(department of Urology,Jiangsu Province Hospital of Chinese Medicine,Affiliated to Nanjing University of Chinese Medicine)from June 2018 to December 2019 were recruited.They were all treated with drugs,extracorporeal shock wave lithotripsy and surgery.The basic information such as gender,age,height,body weight,location of disease,recurrence,analyzed the results of calculi composition and identified the TCM constitution types by using Chinese medicine master Professor Wang Qi’s TCM constitution classification method were collected.The statistical methods were applied to analyze the correlation between adult urinary calculus and TCM constitution types.Result:Adult urinary calculis patients’ TCM constitution types were significantly correlated with their BMI,gender,age and site of the disease,and the differences were statistically significant(P<0.01).The most common types of TCM constitutions in adult patients with urinary calculus in the center were phlegm-dampness(25.5%),dampness-heat(20.9%),yang-deficiency(19.0%)and Yin-deficiency(14.5%),among which male patients were mainly dampness-heat and phlegm-dampness while the female patients were mainly yang-deficiency and phlegm-dampness.The affected patients were mainly middle-aged people,and their constitutions were mostly phlegm-dampness and yang-deficiency.The young patients with urinary calculus were mostly dampness-heat and the elderly patients were yin-deficiency.The main focus located in the upper urinary tract.A total of 998 adult patients with urinary calculus were found to be composed of 23 kinds of calculus composition,including 334 of single ones(33.5%),462 of two compound ones(46.3%),194 of three composition compound ones(19.4%),8 of four composition compound ones(0.2%).The mixed form was more common in clinics.Calcium oxalate monohydrate,calcium oxalate dihydrate,and carbonate apatite were the most common calculus components.Qi-deficiency was more likely to cause infectious calculi in adult patients than type of dampness-heat,phlegm-dampness and yin-deficiency.The recurrence rate of urinary calculi in patients with dampness-heat was higher than that in patients with mild constitution,yang-deficiency,phlegm-dampness and yin-deficiency.Patients with mild constitution were not so likely to relapse.After χ2 test and Bonferroni correction,the differences are statistically significant(P<0.003).Conclusion:There is a clear correlation between adult urinary calculus and TCM constitution.The most common TCM constitutional classification of adult urinary calculi were phlegm-dampness,dampness-heat,yang-deficiency and Yin-deficiency.The recurrence rate of patients with dampness-heat calculi is higher.The patients with mild constitution are not easy to relapse.According to Professor Wang Qi’s theory of ’adjustable constitution’,the doctors can use traditional Chinese medicine and guide the diet and lifestyle of patients with urinary calculus systematically.So as to improve and correct the biased constitution and provide new directions and strategies for the prevention and treatment of adult urinary calculus.
