Objective:To understand the changes of high-risk groups,related risk factors and intervention effect of stroke in hospitals in wuhu area,provide scientific basis for timely adjusting the intervention intensity and measures of risk factors,analyze and summarize the intervention situation of ischemic stroke in this area,and provide scientific basis for relevant departments to better guide the prevention and control work.Methods:A prospective study was conducted to continuously collect patients with acute stroke and patients at high risk of stroke from September 2017 to September 2018 in the second people's hospital of wuhu city,anhui province.Based on the 2017 "program of in-hospital screening and intervention for high-risk groups of stroke",the screening population of 1997 patients in wuhu area was selected as the observation cohort.The changes in the composition of high-risk groups and related risk factors of stroke and the intervention effect were observed after 1 year of follow-up.Group: cerebral infarction group: included patients with cerebral infarction;Intracerebral hemorrhage group: the enrolled patients with intracerebral hemorrhage;High-risk group: patients with previous history of cerebral infarction or cerebral hemorrhage or with 3 or more of the risk factors.Results:(1)a total of 1,997 patients met the inclusion criteria in this study.The subjects were divided into three groups: 780 patients in the cerebral infarction group,218 patients in the cerebral hemorrhage group,and 999 patients in the high-risk group.After1 year of follow-up,28 patients died,including 10 in the cerebral infarction group,3 inthe cerebral hemorrhage group and 15 in the high-risk group.Among the 150 cases,48 cases were lost to follow-up in the cerebral infarction group,12 cases were lost to follow-up in the cerebral hemorrhage group,and 90 cases were lost to follow-up in the high-risk group.(2)The proportion of the high-risk group with hypertension before intervention was significantly higher than that of the cerebral infarction group.The proportion of high-risk group with diabetes history was significantly higher than that of cerebral infarction group and cerebral hemorrhage group.The proportion of patients with a history of hyperlipidemia was in the high-risk group,the cerebral infarction group and the cerebral hemorrhage group.The proportion of overweight or obese patients from the largest to the smallest is cerebral infarction group,high-risk group,cerebral hemorrhage group.There was no significant difference in the history of stroke,exercise,smoking and drinking.(3)the proportion of patients with hypertension before intervention in the high-risk group was significantly higher than that in the cerebral infarction group;The proportion of high-risk group with diabetes history was significantly higher than that of cerebral infarction group and cerebral hemorrhage group.The proportion of patients with a history of hyperlipidemia was in the high-risk group,the cerebral infarction group and the high-risk group.The proportion of overweight or obese patients from the largest to the smallest is cerebral infarction group,high-risk group,cerebral hemorrhage group.There was no significant difference in the history of stroke,exercise,smoking and drinking.(4)Control of risk factors in the three groups: blood pressure,blood sugar,exercise and smoking/drinking were well controlled in the cerebral infarction group;In the cerebral hemorrhage group,the motor control was better,and the blood pressure and blood lipid were not controlled.Blood pressure,blood sugar,exercise and smoking/drinking were well controlled in the highrisk group.(5)after intervention,patients with hypertension,diabetes,lack of exercise,smoking and alcohol consumption in the cerebral infarction group had a higher rate of stroke recurrence;The incidence of recurrent stroke in patients with diabetes afterintervention was higher in the intracerebral hemorrhage group.After intervention,the high-risk group had a higher rate of recurrent stroke in patients with hypertension,diabetes,hyperlipidemia,inactivity,smoking and drinking.(6)Logistic attribution analysis of risk factors for stroke occurrence or recurrence in each group showed that the risk factors for stroke recurrence in the cerebral infarction group after intervention included hypertension,hyperglycemia,smoking and drinking,and the protective factor was keeping exercise.Hyperglycemia was a risk factor for stroke recurrence in the intracerebral hemorrhage group after intervention.The risk factors for stroke recurrence in the high-risk group after intervention included hypertension,diabetes,hyperlipidemia,smoking and alcohol consumption.Conclusion:The risk factors of stroke in hospital patients in wuhu region were different in gender,age and stroke type.After 1 year of intervention,the proportion of risk factors in each research group changed,and the differences and changes of these risk factors reflected the implementation of intervention measures,which is of great significance for the prevention and treatment of cerebral apoplexy and the control of recurrence rate in this region in the future.
