Background and Objectives: Hemoglobin a1c(HbA1c),blood lipid,atherosclerosis are important risk factors for cerebral stroke.At present,there is no clear study on the relationship between HbA1 c level with atherosclerosis and cerebral stroke.Cholesterol(CHOL)is closely related to Atherosclerotic vascular disease(ASVD),but the relationship between triglyceride(TG)and ASVD is controversial.There are differences in the influence factors of anterior circulation and posterior circulation infarction.In this study,clinical data of patients with acute cerebral infarction were collected and analyzed retrospectively.The correlation between HbA1 c,blood lipid,atherosclerosis,and anterior and posterior cerebral infarction was analyzed.Method: Through retrospective analysis of clinical data,the study gets the age,gender,blood pressure at admission,blood glucose,HbA1 c,serum TG,total cholesterol(TC),and low density lipoprotein cholesterol(LDL-C),high density lipoprotein cholesterol(HDL-C),property of carotid plaque,cerebral infarction site,NIHSS score,MRS score,etc.A total of 247 patients were enrolled and grouped according to the property of carotid atherosclerotic plaque,HbA1 c value,and the site of anterior & posterior circulation infarction.ANOVE test and univariate & multivariate logistic regression analysis were used to investigate the correlation among HbA1 c,blood lipid,atherosclerosis,and anterior and posterior cerebral infarction.Results:There are a total of 247 patients(161 males and 86 females)with acute cerebral infarction,including 74 in the stable plaque group and 173 in the unstable plaque group.According to the Logstic regression analysis,the hypertension(OR=2.167,P=0.008)and the age(OR=1.043,P=0.001),as risk factors,result to significant differences.For every 1% increase in HbA1 c,the risk of developing carotid artery plaque in diabetic patients increases by 2.6%,which is a risk factor for atherosclerosis(OR=1.026,P=0.771).There are 146 cases with HbA1c≤6.0%,50 cases with 6.0%<HbA1c<7.0%,and 51 cases with HbA1c≥7.0%.There are significant differences in their TG(Z = 8.750,P = 0.000)and HDL-C(F = 6.664,P = 0.002).In multiple regression analysis,for every 1mmol / L increase in blood glucose,HbA1 c increases by 0.545%(t = 22.087,P <0.001).And for every 1mmol/L increase in HDL-C,HbA1 c decreases by 0.508%(t=-2.877,P=0.004).There are 143 cases of anterior circulation infarction group(85 males,58 females),104 cases of posterior circulation infarction group(76 males,28 females).Logstic regression analysis reveals significant differences in risk factors for posterior circulation infarction among males(OR=1.852,P=0.027).For each 1% increase in HbA1 c value,the risk of postcyclic infarction in diabetic patients increases by 7.2%(OR=1.852,P=0.386).Conclusion: The increasing of HbA1 c will beget atherosclerosis.With the raise of HbA1 c,TG will increase and HDL-C will decrease.As the higher the rise of HbA1 c,the greater the incidence of acute post-circulatory ischemic disorder is.
