Study On Serum Levels Of SBDPs And Their Clinical Significance In Patients With Spinal Cord Injury

Objective:In this study,the diagnostic value of SBDPs in serum for spinal cord injury was investigated by comparing the relevant clinical data of patients with spinal cord injury and the expression level of SBDPs in serum.Methods:Patients with acute spinal fractures who were admitted to the emergency department of subei people’s hospital from September 2018 to December 2019 were selected and the data were complete.Patients were divided into three groups according to the international standard for neurological classification of spinal cord injury(AIS classification)published by the American spinal cord injury association: control group(AIS classification E),mild spinal cord injury group(AIS classification C and D),and severe spinal cord injury group(AIS classification A and B).Fifteen cases were randomly selected from each group,and a total of 45 cases were included in the study.After admission,serum of the patient was extracted within 24 hours after injury and the SBDPs level and TNF-α level(tumor necrosis factor-α,TNF-α)in the blood were detected.Multivariate quantitative data of normal distribution were compared using the variance analysis,multivariate quantitative data of non-normal distribution were compared using the Kruskal–Wallis analysis.The multivariate qualitative data(gender,personal history)were compared using χ2 test.Regrouping according to whether there is nerve injury or not(SCI mild injury group +SCI severe injury group VS control group).Subject operating characteristic curve(ROC curve)was drawn for relevant indicators(SBDPs level and TNF-α levels),and relevant indicators such as cut-off value,sensitivity,specificity and area under the curve were calculated.When P<0.05,the difference was statistically significant.Results:1、Clinical dataThere were no significant differences in the basic data(gender,age,body mass index,complications and personal history)of enrolled patients.2、Expression level of SBDPs in serum of patients in each groupThe expression levels of SBDP120 in serum of patients in the control group,SCI mild injury group and SCI severe injury group were(5.131±0.955),(5.420±1.048)and(5.461±0.900)ng/ml,respectively.There was no statistical significance between the groups through variance analysis(F=0.519,P=0.599).The expression levels of SBDP145 in serum of patients in the control group,SCI mild injury group and SCI severe injury group were(7.756±1.656)、(9.122±0.910)and(9.878±0.949)ng/ml,respectively.There were statistically significant differences among the groups through variance analysis(F=11.632,P<0.001).In multiple comparisons,SBDP145 level in serum of patients in SCI severe injury group and SCI mild injury group was significantly higher than that in control group(P<0.05).There was no statistical difference between the SCI mild injury group and the SCI severe injury group(P<0.097).3、Expression level of TNF-α in serum of patients in each groupTNF-α was first discovered because of its ability to kill tumor cells.With the development of studies,it has been found that TNF-α has some potential value in the diagnosis of central nervous system injury.Therefore,TNF-α was used as a reference index to evaluate the diagnostic value of SBDPs in SCI by comparison.Therefore,the expression level of TNF-α in serum of patients was also detected.The expression levels of TNF-α in serum of patients in the control group,SCI mild injury group and SCI severe injury group were 1.754(1.642,1.841)、 1.896(1.694,1.939)and 1.902(1.864,2.004)ng/ml,respectively,and there were statistically significant differences among the groups through Kruskal–Wallis analysis(F=11.922,P=0.003).In multiple comparisons,TNF-α level in serum of patients in SCI severe injury group and SCI mild injury group was significantly higher than that in control group(P<0.05).There was no statistical difference between the SCI mild injury group and the SCI severe injury group(P<0.160).4、ROC curve analysis of the expression levels of SBDP145 and TNF-α in serumIn order to explore the diagnostic significance of SBDP145 expression level in serum for SCI,ROC curve analysis was used.The Area Under the receiver operating characteristic Curve(AUC)for the diagnosis of SBDP145 expression level in serum was 0.840,the cutoff value was 8.445 ng /ml,the sensitivity was 90.0%,the specificity was 73.3%,and yoden index was 0.633.The AUC for diagnosis of TNF-α Expression level in serum was 0.791,cut-off value was1.857ng/ml,sensitivity was 70.0%,specificity was 86.7%,and yoden index was0.567.The results showed that the expression levels of SBDP145 and TNF-α in serum had potential value in diagnosing SCI,and SBDP145 had greater potential value.Conclusion:This study shows that there is a statistical difference in the specific increase of SBDP145 expression level in serum of patients after acute SCI,which has certain potential value for early diagnosis of SCI.
