| Objective Heart failure(HF)is the clinical common disease and frequently-occurring disease,it is a variety of cardiovascular diseases,such as hypertension,coronary heart disease,enlarge heart disease,valvular disease,etc.the end-stage of syndrome.Its incidence has been rise,and a serious threat to human survival and health,early to take effective means of diagnosis in patients with HF on the early intervention treatment can yet be regarded as a kind of effective method,and the detection is a marker of heart failure guidelines recommended by the most effective method at home and abroad.This topic to study and put forward a new marker s ST2 HF can be used as a marker of heart failure independent index,and the joint monitoring HF markers n-terminal brain natriuretic peptide precursor(NT-pro BNP),B-type natriuretic peptide(BNP),soluble matrix cracking element(s ST2),the method of analysis of the three markers of its relationship with HF HF,to explore the clinical value of three markers for diagnosis of HF;The other USES the follow-up study to observe three markers in patients with HF HF mortality and readmission rate value judgment,to compare the value of the three markers on the prognosis of HF.Methods 153 HF patients group(HF)is divided into levelsⅡ(n=73),Ⅲ(n=56),Ⅳclass(n=24),were determined every NT-pro BNP in patients with heart failure markers blood,BNP,s ST2 level,the other on the choice of the control group(n=75)with healthy check-up.All selected patients with HF with clinical follow-up way for studying the observed follow-up of 1 year.Statistics and analysis of three types of heart failure between markers in patients with different cardiac function classification of HF,the horizontal difference between HF group and control group;Logistic regression analysis of risk factors for HF,NT-pro BNP ROC curve analysis,the BNP,s ST2 value to the diagnosis of HF.HF patients were followed up for 1 year,comparing three markers in patients with HF HF markers predictive value and analyzes three kinds of heart failure and the correlation of death in patients with HF.Results Compared with control group,the blood NT-pro BNP,BNP HF group and s ST2 level increased significantly(P<0.01).With the increase of cardiac function classification,three types of heart failure in HF patients blood markers level also will increase accordingly(P<0.05).Single factor regression analysis showed that the correlation of three markers and heart failure in heart failure was statistically significant(P < 0.01);Multiple factors regression analysis showed that the NT-pro BNP,BNP may be independent risk factors for heart failure(P < 0.01).ROC results show that s ST2 diagnosis of HF area under the ROC curve slightly higher than the NT-pro BNP,BNP,0.921,0.908,0.890 respectively(P<0.05).1 year follow-up study,according to the results of 11 cases died of heart failure patients,accounted for 7.19%;1 year three types of death in patients with blood marker levels were significantly higher than those not death in patients with heart failure(P<0.05);Multiariable Cox regression model analysis showed that patients with baseline obvious correlation markers level three types of heart failure and death(OR = 1.86);SST2 for judgment of prognosis area under the ROC curve(AUC)= 0.792 and the BNP(AUC=0.778),the NT-pro BNP similar(AUC=0.801),and the combined application of prediction effect(AUC= 0.859)increased significantly.Conclusions s ST2 can be used as a marker of heart failure independent index,s ST2 than other markers in the diagnosis of heart failure and prognosis value judgment better has its unique advantages.NT-pro BNP,BNP,s ST2 clinical value for diagnosis of HF,and mortality in patients with HF and readmission rate for more effective diagnosis value of prognostic value in patients with HF can be more systematic and comprehensive evaluation. |
