With the arrival of the silver wave,the trend of population aging and aging is increasingly obvious.The decline of physical function and health level of the elderly directly increases the prevalence and morbidity of the elderly,which will inevitably lead to a significant increase in their demand for medical services,thus entering the peak period of medical service demand and utilization.As can be seen from the data of the recent three national health service surveys,the proportion of the elderly who should be admitted to hospital but are not,and the proportion of the elderly who should be admitted to hospital but are not admitted to hospital are all higher than that of the young adults.The latest survey shows that the proportion of the elderly who should be admitted to hospital but are not admitted to hospital is more than twice that of the young adults,and the utilization of medical services for the elderly is obviously insufficient.In this context,this article will old people as the research object,using the China health and pension track(CHARLS)in the third phase of cross section data,to the elderly aged over 60 one full year of life as the research subject,with the influencing factors of elderly health service utilization as the research content,research the long-term significant factors affecting the elderly health service utilization,based on this,a reasonable put forward measures to improve the level of the elderly health service utilization.Based on Andersen’s medical service utilization model and Grossman’s health and needs theories,this paper proposes research hypotheses.This study used relevant data from the health service survey once every five years to investigate the current situation of medical service utilization of the elderly in China,and illustrated the gap between the elderly group and the national medical service utilization with the comparative analysis method,which reflected the huge gap between the medical service needs of the elderly and the actual utilization of medical services.Secondly,based on the consolidation of stage 3 questionnaire,to our country the elderly health service utilization is divided into two stages,using the two models,binary Logit model and multiple linear regression method,statistics describe the elderly outpatient service utilization,inpatient services use two aspects in different orientation characteristics,distribution changes of ability factor,demand factor,and make use of the two models will be the old medical treatment is divided into two stages: medical decision-makingbehavior and medical out-of-pocket costs,factors affecting research were carried out respectively,and compare the influence factors of different periods,and find out the long core factors affecting the elderly health service utilization.Finally,the following conclusions are drawn: the medical service utilization of the elderly is affected by various factors,and the influencing factors are not exactly the same in each period,and the same influencing factors will have great or even opposite changes in the degree and effect of the same influencing factors on the medical service utilization in different periods.No matter it is the inpatient model or the outpatient model,the complete social security and the chronic diseases almost all have significant influence.The complete social security has significant negative influence on the unvisited rate of medical treatment,the in-patient rate,the out-of-pocket expense of outpatient service and the out-of-pocket expense of hospitalization.Chronic diseases have a significant positive effect on outpatient and hospitalization out-of-pocket expenses.Finally,based on the above research conclusions,this paper proposes that to improve the utilization level of medical services for the elderly,we should start from the following aspects: reducing the proportion of personal self-payment and increasing financial input;To advocate healthy behavior and pay attention to prevention in advance;We will improve social security policies and raise the level of reimbursement;Establish health records for the elderly to prevent and treat chronic diseases.
