| Objective:By comparing and analyzing the clinical and pathological characteristics of the patients with left-and right-sided colon cancer,the similarities and differences are studied to provide evidence for the two diseases theory.Method:All clinical data of 190 colon cancer patients(142 and 48 cases of right-sided and left-sided colon cancers respectively),who received radical surgery treatment and was confirmed by postoperative pathology in Shandong Qilu hospital during June 2018 to June 2019.were collected,including gender,age,BMI,drinking history,smoking history,diabetes history,clinical symptoms,blood CEA,blood CA19-9,tumor maximum diameter,pathological type,degree of differentiation,tumor general type,involvement of lymph nodes,tumor TNM stage,number of tumor deposits,presence of lymphovascular invasion and presence of perineural invasion,etc.And the chi-square test and non-parametric test were used for statistical analysis of the above data.Results:1.It was demonstrated that age,gender,drinking history,smoking history,diabetes history,blood CEA and CA19-9 levels,general tumor type.tumor T stage,tumor deposits,presence of lymphovascular invasion and presence of perineural invasion had no significant correlations with tumor anatomical locations.2.The proportion of patients with BMI≥28.0Kg/m2 in left-sided colon cancer group was 16.7%,significantly higher than 5.7%of right-sided colon cancer group(P<0.05).3.Svmntoms related to anemia were more common in patients with right-sided colon cancer,while bloody stools or black stools and ileus were more frequently associated with patients with left-sided colon cancer.The difference was statistically significant(P<0.05).4.The tumor diameters of 72.1%right-sided colon cancer and 50.0%left-sided colon cancer were more than 5cm.The difference was statistically significant(P=0.005)5.The proportion of right-sided colon cancer(43.0%)in histological grades 3 and 4 was significantly higher than that of left-sided colon cancer(18.8%)(P=0.010).6.Tumors with less than 12 lymph nodes resections were more common in left-sided colon cancer group than in right-sided group,and difference was statistically significant(P=0.026).7.The right-sided colon cancer was significantly associated with a lesser advanced N stage(P=0.014).The proportions of tumors in N0,N1,and N2 stages were 69.7%,23.2%,and 7.0%in right-sided group,and 50.0%,37.5%,12.5%in left-sided group.Instead,left-sided colon cancer had a more advanced TNM stage than right-sided colon cancer did.The proportion of patients with stage III tumor in left-and right-sided group were 50.0%and 30.3%respectively,and the difference was statistically significant(P≤0.05).Conclusions:1.Obese patients have a higher risk of left-sided colon cancers than right-sided colon cancers.2.Compared with right-sided colon cancer,left-sided colon cancer has a lower histological grade,higher rate of lymph node metastasis,and higher TNM tumor stage. |
