| Objective Detecting the levels of C3 in the serum of patients with primary angle-closure glaucoma and the healthy population in order to verify whether the change of serum C3 level in patients with primary angle-closure glaucoma is related to the disease.Through the examination of intraocular pressure,retinal nerve fiber layer thickness and visual field defect in patients with primary angle-closure glaucoma.To investigate whether the serum C3 level of patients with primary angle-closure glaucoma is related to intraocular pressure,retinal nerve fiber layer thickness and degree of visual field defect.And to explore the significance of serum C3 levels and the incidence of primary angle-closure glaucoma.Methods Based on the diagnostic criteria for primary angle-closure glaucoma in China,32 patients with primary acute angle-closure glaucoma who were hospitalized and prepared for surgery were selected after excluding the relevant influencing factors.24 healthy people who were examined in our hospital medical center at were selected at the same period.All patients were measured and recorded their visual acuity,intraocular pressure,chamber angle,refractive state,optic disc OCT,visual field and so on.The optic disc OCT examination uses Glaucoma Module Premium Edition Software(GMPE)with a circular scan with a 3.5 mm radius around the optic disc to get the retinal ganglion cell thickness parameter which automatic correction,then divided patients into normal group,critical group and abnormal group.Visual field inspection uses the center 30-2,SITA Standard to inspect,and divided into mild visual field defect group(MD>-6dB),moderate visual field defect group(-12dB<MD<-6dB),and severe visual field defect group(MD>-12dB).In the early morning of the second day after admission,fasting blood was collected and sent to the laboratory.The serum C3 content was determined by immunoturbidimetry.In all cases,"compound trabeculectomy" was performed by experienced doctors under local anesthesia.Three days after the operation,the intraocular pressure was measured to be stable.All the control group was collected blood by the routine physical examination procedure and detected serum C3 content.The data was analyzed using SPSS 21.0 software,and the results were expressed as mean± standard deviation(x±s).The serum C3 levels of the PACG preoperative group and the PACG postoperative group were analyzed by paired sample t test,and independent samples t test were used for the PACG group and the control group for comparison.Correlation analysis of serum complement C3 level with age,gender,intraocular pressure,the degree of thinning of the optic nerve fiber layer and the degree of visual field defect using Spearman correlation analysis.Results Serum C3 levels in the PACG preoperative group,the PACG postoperative group,and the control group were 1.033 ±0.147g/L,1.049±0.154g/L,and 1.150±0.165g/L.Compared with the control group,the serum C3 levels of the PACG preoperative group and the PACG postoperative group were significantly different(P<0.05).The serum C3 levels of the PACG preoperative group and the PACG postoperative group were also statistically difference(P<0.05).There were no significant correlation between serum complement C3 level and age and gender in the PACG group(r=0.017;r=-0.147,P>0.05).There were no significant correlation between serum complement C3 level and age and gender in the control group(r=-0.161;r=0.110,P>0.05).There was a significant negative correlation between intraocular pressure and serum complement C3 level in the preoperative group of PACG(r=-0.605,P<0.05),and there were no significant correlation between intraocular pressure and serum complement C3 level in the post-PACG group(r=-0.147,P>0.05).The retinal nerve fiber layer thickness and visual field defect in PACG group were significantly negatively correlated with serum C3 level(r=-0.556;r=-0.641,P<0.05)Conclusion Healthy human serum C3 levels are not affected by age and gender.Patients with primary angle-closure glaucoma have significantly lower serum C3 levels compared to normal human.After surgery,serum C3 levels can increase,but can not increase to the normal level in the short term.Serum C3 levels in patients with primary angle-closure glaucoma are inversely related to intraocular pressure,retinal nerve fiber layer thickness and visual field defect.Detection of serum C3 levels can reflect the degree of damage of primary angle-closure glaucoma.Figure 5 table 4 reference 56... |
