Effect Of Subchorionic Hematoma On Pregnancy Outcome Of Threatened Abortion And Analysis Of Related Risk Factors

Objective:Through the analysis of the clinical characteristics and pregnancy outcome of threatened abortion patients with and without subchorionic hematoma(simple threatened abortion),we discussed the influence of SCH on pregnancy outcome of threatened abortion patients.And the relationship between SCH related factors and pregnancy outcome was further analyzed to improve the understanding of SCH high risk factors.Methods:We collected the relevant clinical data of the patients with threatened abortion who were hospitalized in the provincial hospital affiliated to Shandong University hospital from July 2017 to May 2019,a total of 199 cases were included.We used retrospective case-control study method and collected relevant clinical data.According to whether combined with SCH,we divided them into two groups:combined with SCH threatened abortion 87 cases as case group,not combined with SCH patients(simple threatened abortion patients)112 cases as control group.First,we compared and analyzed the clinical features and pregnancy outcome of the two groups.Then,we analyzed the high risk factors of threatened abortion patients with SCH,mainly from the aspects of pregnant women’s age,placental location,SCH diagnosis gestational weeks,hematoma size,hematoma location,hematoma duration and clinical symptoms to explore the high risk factors affecting pregnancy outcome in patients with SCH threatened abortion.P<0.05 was considered that the difference was statistically significant.Result:1.Comparison of pregnancy outcomes between case group and control group:(1)The gestational weeks of admission were 15.38±3.82 weeks,the days of hospitalization were 7.09±5.33days in case group;the gestational weeks of admission were 18.05±5.88 weeks,the days of hospitalization were 4.22±3.21 days in control group.The difference between the two groups was statistically significant during gestational weeks and days of hospitalization.(2)The abortion rate was 21.8%,the premature delivery rate was 9.2%and the term delivery rate was 69%in case group;the abortion rate was 6.3%,the premature delivery rate was 12.5%and the term delivery rate was 81.3%in control group;the difference of abortion rate and term delivery rate was statistically significant,but the difference of premature delivery rate was not statistically significant between the two group patients.2.Single-factor analysis of risk factors in patients with SCH threatened abortion:(1)The abortion rate was 37.5%,the premature delivery rate was 9.4%and the term delivery rate was 53.1%in the patients whose gestational weeks≤14 weeks diagnosed with SCH;the abortion rate was 12.7%,the premature delivery rate was 9.1%and the term delivery rate was 78.2%in the patients whose gestational weeks>14 weeks diagnosed with SCH,the difference of abortion rate and term delivery rate was statistically significant,but the difference of premature delivery rate was not statistically significant.(2)The abortion rate of patients whose hematoma volume was<30ml,30~60ml and>60ml was 12.5%,35.7%and 66.7%respectively.,and the difference was statistically significant.(3)The abortion rate was 62.1%,the premature delivery rate was 6.9%and the term delivery rate was 31%in patients with persistent subchorionic hematoma;The abortion rate was 1.7%,the premature delivery rate was 10.3%and the term delivery rate was 87.9%in patients with non-continuous subchorionic hematoma;the difference of abortion rate and term delivery rate was statistically significant,but the difference of premature delivery rate was not statistically significant between the two group patients.(4)The abortion rate was 41.7%,the premature delivery rate was 8.3%and the term delivery rate was 50%in patients with abdominal pain and vaginal bleeding;the abortion rate was 14.3%,the premature delivery rate was 9.5%and the term delivery rate was 76.2%in patients with abdominal pain or vaginal bleeding;the difference of abortion rate and term delivery rate was statistically significant,but the difference of premature delivery rate was not statistically significant between the two group patients.Conclusion:1.The abortion rate of threatened abortion patients with SCH is significantly higher than that of simple threatened abortion patients,while the rate of full term delivery is lower than that of simple threatened abortion group and the difference of premature delivery rate is not obvious.The threatened abortion patients with SCH are admitted into hospital earlier and the days of hospitalization are longer than the simple threatened abortion patients.2.The smaller gestational weeks of diagnosis,the larger hematoma volume,persistent subchorionic hematoma and the presence of abdominal pain and vaginal bleeding are independent risk factors for adverse pregnancy outcomes in patients with SCH threatened abortion.
