Objective:The purpose of this study was to investigate the relationship between vocal cord polyp recurrence and Laryngopharyngeal reflux,to clarify the role of pharyngeal regurgitation in vocal cord polyp recurrence,to further guide vocal cord polyp postoperative medication,to reduce the possibility of vocal cord polyp recurrence,and to improve patient voice quality.Methods:1.The group of recurrent vocal cord polyps,who came to our hospital from December 2017 to December 2019 due to hoarseness,had a history of vocal cord mass surgery,and postoperative medical examinations suggested vocal cord polyps.Laryngoscopy showed vocal cord masses,which had not been processed by a proton pump inhibitor for the past eight weeks.Twenty-one patients with vocal cord polyps were still underwent resection of the vocal cord masses under support laryngoscope in our hospital.2.Vocal cord polyp group:Patients with hoarseness who were hospitalized in the Department of Otorhinolaryngology Head and Neck Surgery of the Second Affiliated Hospital of Kunming Medical University from December 2017 to December 2019 were selected.No previous history of vocal cord mass surgery,electronic laryngoscope examination showed vocal cord mass 60 cases of vocal cord mass resection under supportive laryngoscope were hospitalized,and 60 cases of vocal cord polyps were diagnosed after surgery.3.Control group:randomly selected 30 trainee students who participated voluntarily,had a history of hoarseness,vocal cord mass,and underwent voluntary laryngoscope examinationThe above subjects completed the Reflux Symptom Index Scale(RSI)taken into account their symptoms and the severity of their symptoms under the guidance of a physician.At the same time,two senior doctors based on the Reflux Signs Rating Scale(RFS).The subject's electronic laryngoscope was scored,and the initial diagnosis of RS1 greater than 13 points and RFS greater than 7 points was throat reflux.At the same time,relevant factors(smoking,drinking,excessive voice use)that may cause vocal cord polyps recurrence were incorporated into the statistics..Results:Among 21 patients with recurrence of vocal cord polyps,the proportion of smokers was 47.6%,which was higher than that of the normal group(20.0%)(?2=4.377,p<0.05).The difference was statistically significant.The proportion of patients who consumed alcohol in the case group was 14.3%,which was higher than that in the normal group(0.0%)(X2=4.554,p<0.05).The difference was statistically significant.The proportion of throat reflux in the case group was 42.9%,which was higher than that in the normal group of 6.7%(?2=9.564,p<0.05),and the difference was statistically significant.The cases with excessive and inappropriate voices accounted for 52.4%in the case group,which was higher than that in the normal group with 3.3%(?2=16.516,p<0.05).The difference was statistically significant.Therefore,smoking,drinking,excessive or improper use of throat,and throat reflux was included in the variables of the binary logistic regression model for multivariate analysis of recurrent vocal cord polyps.Throat reflux OR=15.888,p<0.05,which indicates that throat reflux is a risk factor for the occurrence of recurrent vocal cord polyps,and the occurrence of recurrent vocal cord polyps in throat reflux is 15.888 times that of non-laryngeal reflux.Smoking?drinking?excessive or improper vocalization were significant p were all greater than 0.05(p values were 0.999,0.999,0.999 respectively),indicating smoking,drinking,excessive vocalization or improper vocalization Can not significantly affect the occurrence of recurrent vocal cord polyps.Among 60 patients with vocal cord polyps,smoking patients accounted for 41.7%,which was larger than the normal group smoking patients accounted for 20.0%(?2=4.158,p<0.05),the difference was statistically significant.The proportion of patients in the case group who consumed alcohol was 15.0%,which was higher than that in the normal group who accounted for 0.0%(?2=5.000,p<0.05).The difference was statistically significant.The proportion of patients with throat reflux in the case group was responsible for 40.0%,which was 6.7%higher than that in the normal group(?2=10.817,p<0.05),and the difference was statistically significant.Excessive or improper use of the voice in the case group accounted for 30.3%,which was higher than that in the normal group 3.3%(?2=8.540,p<0.05).The difference was statistically significant.Therefore,smoking,drinking,excessive or improper vocalization,and throat reflux were contained in the binary logistic regression model variables for multivariate analysis of vocal cord polyp occurrence.Throat reflux OR=10.404,p<0.05,indicating that throat reflux is a danger factor for the occurrence of vocal cord polyps,and vocal cord polyps that occur in throat reflux are 10.404 times more non-laryngeal reflux.Smoking?drinking?excessive or improper vocalization Significant p were all greater than 0.05(p values were 0.794,0.999,0.072),indicating that smoking,drinking,excessive vocalization,or improper vocalization can not significantly affect the occurrence of vocal cord polyps.Through the above analysis,it was concluded that throat reflux is an independent influencing factor of vocal cord polyps.Conclusion:Throat reflux is not only a related factor for postoperative recurrence of vocal cord polyps,but also an independent risk factor for postoperative recurrence of vocal cord polyps;throat reflux is also an independent risk factor for the occurrence of vocal cord polyps,and compared to vocal cord polyps Occurrence,throat reflux is highly correlated in postoperative recurrence of vocal cord polyps.
