Changes Of Serum Biochemical Indexes And Blood Routine In Patients With Parkinson’s Disease And Analysis Of Risk Factors

Objective: The changes of serum total bilirubin(TBIL),Neutrophil/Lymphocyte Ratio(NLR),serum uric acid/creatinine(UA/Cr),Apolipoprotein AI(ApoAI),Apolipoprotein AI/high-density lipoprotein(ApoAI/HDL),Apolipoprotein B(ApoB),serum myoenzyme(CK,CK-MB,LDH),hemoglobin(Hb),hematokrit(HCT),Cystatin C(Cysc)and Homocysteine(Hcy)between Parkinson’s disease(PD)group and control group were observed and analyzed,and the independent influcing factors were discussed.Further draw a nomogram model to predict the possible risk of PD,and provide a basis for the early diagnosis and prevention of PD.Methods: 251 PD patients admitted to the department of neurology of the second hospital of Lanzhou University from January 2016 to November 2019 were retrospectively collected as the case group,and were divided into groups according to gender,age and H-Y stages.During the same period,220 healthy outpatients of neurology with no significant difference in gender and age from the case group and no obvious abnormalities in neurological imaging examination and physical examination were collected as the control group,and they were also grouped according to gender and age.To analyze and compare the change characteristics of serological indexes between PD group and control group as well as between different subgroups,analyze the independent influcing factors of PD,then the nomogram model was drawn to predict the risk of PD.Results:(1)UA/Cr,Hb,HCT,ApoB,ApoAI/HDL in the PD group were lower than those in the healthy control group,while TBIL,Hcy,LDH and NLR were higher than those in the control group,with statistically significant differences(P< 0.05).Ck-MB,ApoAI and CysC showed no significant difference between the two groups(P>0.05).(2)PD group and control group of different genders: The levels of UA/Cr,HCT,ApoB and ApoAI/HDL in the male PD group were lower than those in the male control group,while the levels of TBIL and Hcy were higher than those in the male control group,with statistically significant differences(P<0.05),however,no statistical difference was found between two groups in the levels of other indicators(P>0.05).The level of UA/Cr and the level of ApoB in the female control group were higher than those in the female PD group,while the levels of Hcy,LDH and NLR in the female control group were lower than those in the female PD group,and the difference was statistically significant(P<0.05),however,no statistical difference was found between two groups in the levels of other indicators(P>0.05).(3)PD group and control group of different ages: The levels of UA/Cr,ApoB and ApoAI/HDL in the young and middle-aged PD group were lower than those in the young and middle-aged control group,while the levels of TBIL,Hcy,CK-MB,LDH and NLR in the young and middle-aged PD group were higher than those in the young and middle-aged control group,with statistically significant differences(P<0.05),however,no statistical difference was found between two groups in the levels of other indicators(P>0.05).The levels of UA/Cr,CK,ApoB and ApoAI/HDL in the elderly PD group were lower than those in the elderly control group,while the levels of Hcy and NLR were higher than those in the elderly control group,with statistically significant differences(P<0.05),however,no statistical difference was found between two groups in the levels of other indicators(P>0.05).(4)The levels of UA/Cr,CysC and ApoAI/HDL were not significantly different between the early and middle PD groups(P>0.05),but the levels of these indicators in the late PD group were significantly lower than those in the early and middle PD groups,with statistically significant differences(P<0.05).There were no significant differences in LDH and NLR levels between the early and middle PD groups,but the levels of the above indicators in the late PD group were significantly higher than those in the early and middle PD groups,with statistically significant differences(P<0.05).There was no significant difference in ApoAI level between the middle and late PD groups,but the ApoAI level in the early PD group was lower than that in the middle and late PD groups,and the difference was statistically significant(P<0.05).However,no statistical difference was found between three groups in the levels of other indicators(P>0.05).(5)The results of binary multivariate logistic regression analysis showed that the decrease of UA/Cr,Hb and ApoB levels and the increase of TBIL and Hcy levels may be independent risk factors for PD(P<0.05).For every unit increase in TBIL,the risk of PD increased by 10.49%.For every one unit increase in Hcy,the risk of PD increased by 10.71%.(6)The R software was used to establish a nomogram prediction model of personalized PD disease.The results showed that with the increase of TBIL and Hcy,UA/Cr,Hb and ApoB decreased,the score of nomogram model increased,and the risk of PD increased correspondingly.Conclusions:(1)Serum UA/Cr,Hb,HCT,ApoB,ApoAI/HDL decreased in PD patients,and UA/Cr and ApoB decreased in PD patients of different ages and genders.Serum TBIL,Hcy,LDH and NLR were increased in PD patients,and Hcy was increased in PD patients of different ages and genders.(2)Serum UA/Cr,CysC and ApoAI/HDL levels in patients with advanced PD were lower than those in patients with early and middle PD;the serum levels of NLR in patients with advanced PD were higher than those in patients with early and middle PD.(3)The decrease of UA/Cr,Hb and ApoB levels and the increase of TBIL and Hcy levels may be independent risk factors for PD.
