Objective:Hepatocellular carcinoma is a highly malignant tumor associated with a poor prognosis,and the molecular mechanisms remain poorly understood.KIAA1522 expression is upregulated in various types of tumor tissue;however,its function remains unknown in HCC.This article is to study the expression of KIAA1522 gene in primary hepatocellular carcinoma and adjacent tissues,and further study the relationship between KIAA1522 gene and the prognosis of hepatocellular carcinoma patients,so as to provide a basis for the early diagnosis,effective treatment and prognosis assessment of hepatocellular carcinoma.Methods:(1)The expression of KIAA1522 gene in Oncomine,Oncolnc and other databases and its relationship with clinical prognosis were analyzed by bioinformatics.(2)79 cases of primary hepatocellular carcinoma in hepatobiliary surgery department of our hospital in 2013 were collected and their relevant clinical data were obtained.Meanwhile,79 cases of pathological specimens were obtained from the pathology department.Total RNA was extracted from the collected samples of 79 pairs of hepatocellular carcinoma and para-carcinoma tissues,and the expression level of KIAA1522 gene was detected by realtime quantitative PCR.(3)Tissue microchip-immunohistochemical staining was used to detect the expression of KIAA1522 protein in 79 cases of primary hepatocellular carcinoma and their paired para-carcinoma tissues,and the relationship between its positive expression rate and the prognosis of patients was statistically analyzed.(4)The relationship between KIAA1522 expression and clinicopathological indicators in hepatocellular carcinoma was analyzed by statistical method.(5)Univariate and multivariate analyses of prognosis(COX risk regression model)were performed to find independent risk factors associated with the prognosis of hepatocellular carcinoma.Results:(1)In the Oncomine database,the expression levels of KIAA1522 in hepatocellular carcinoma and para-carcinoma tissues were significantly different(P<0.001),and the postoperative survival of patients with hepatocellular carcinoma with high expression of KIAA1522 in the Oncolnc database was significantly shortened(P<0.05).(2)The real-time quantitative PCR results showed that KIAA1522 expression in HCC tissues was significantly higher than that in para-carcinoma tissues(P<0.05).(3)Immunohistochemical staining showed that the positive expression rate of KIAA1522 protein in para-carcinoma tissues was 15.2%(12/79),while the positive expression rate of KIAA1522 protein in primary hepatocellular carcinoma tissues was 89.8%(71/79),and the difference was statistically significant(P<0.05).Kaplan-Meier survival analysis showed that the positive expression of KIAA1522 protein was positively correlated with the shortened overall-survival time of patients with hepatocellular carcinoma 3 years after surgery(P<0.001,log-rank test).At the same time,this study compared the expression of KIAA1522 in hepatocellular carcinoma with the postoperative recurrence time,and the results showed that the expression of KIAA1522 was independent of the disease-free survival time of patients after the surgery(P>0.05).(4)The results of the study on the relationship between KIAA1522 and clinicopathological factors showed that the expression level of KIAA1522 in hepatocellular carcinoma was independent of the patient’s age,gender,HBV infection,cirrhosis,AFP level,Child-pugh grade,tumor number,tumor size,differentiation,vascular thrombus,hepatic capsule invasion,and TNM clinical stage.(5)Multivariate Cox regression analysis showed that KIAA1522,tumor size and TNM stage were independent predictors of survival in patients with hepatocellular carcinoma(P<0.05).Conclusion: The expression of KIAA1522 in hepatocellular carcinoma tissues was significantly higher than that in para-carcinoma tissues,and the overall survival time was shorter in the group with high expression of KIAA1522,suggesting that the expression of KIAA1522 was closely related to the prognosis of hepatocellular carcinoma patients.Meanwhile,KIAA1522,TNM stage and tumor size were independent risk factors affecting the prognosis of hepatocellular carcinoma patients.All of these suggest that KIAA1522 may serve as a potential molecular marker for predicting the prognosis of patients with primary hepatocellular carcinoma.
