| Objectives Bladder cancer,as one of the most common tumor in urinary system,the incidence increased year by year trend,early bladder cancer often choose transurethral bladder tumor cutting operation,postoperative adjuvant chemotherapy drug perfusion of bladder can achieve good treatment effect,even to achieve clinical cure,and bladder tumor in the event of muscular layer infiltration,the radical cystectomy and urinary diersion technique has become a necessary way of operation,not only increased risk of surgery,also greatly reduces the quality of life of patients with bladder cancer,so looking for an early indicators to identify early treatment is especially important for patients with bladder cancer.Serum interleukin-6(il-6),as a cytokine,plays an important role in the occurrence and development of tumors.It has been proved that it is closely related to a variety of tumors,such as prostate cancer,breast cancer and ovarian cancer,and is related to the growth and invasion of tumors.High levels of il-6 suggest late stage and poor prognosis.However,its research in bladder cancer is still lacking.Our study is to explore the effect of il-6 level on the prognosis of patients with radical cystectomy,to find indicators for early identification of bladder cancer and further to find potential targets for the treatment of bladder cancer Methods Collected 124 cases of 2011 ~ 2012 in Qingdao university hospital uropoiesis surgical department radical cystectomy in patients with clinical and pathological data of exclusion of preoperative merge infection,the other tumors,the combining immune system disease patients,on the surgery day vacuum blood collection needle 2 ml fresh fasting venous blood,using enzyme-linked immunosorbent assay to determine itspreoperative serum level of IL-6,according to the median level of IL-6 is divided into low and high level group,and follow-up of patients with statistical postoperative survival time,using t test and chi-square test comparing the difference between the clinical pathological features between the two groups,Kaplan-meier survival analysis and Cox proportional risk regression model were used to analyze the influence of serum il-6 level on the prognosis of patients.Results 1.The low level to high level group median age was 63.9 + /-11.8 years,respectively,62.8 + /-11.9 years,two groups of IL-6 in a level respectively(13.25 +5.46)pg/ml(7.14-0.92)and pg/ml,T staging in patients with more similar between the two groups have statistical significance(P < 0.05),age,gender,BMI,whether merger carcinoma in situ,smoking,hypertension,diabetes,ECOG score,there was no statistically significant difference comparing adjuvant therapy,etc.2.In kaplan-meier survival analysis,the total survival time of the high-level group was significantly lower than that of the low-level group,and the difference was statistically significant(P<0.05).The 3-year overall survival rate was 85.1% in the low-level group and 51.8% in the high-level group.The 5-year overall survival rate was 70.7% in the low-level group and 40.5% in the high-level group.3.Cox multivariate proportional risk regression model analysis showed that: gender,age,BMI,lymph node metastasis,smoking,alcohol consumption,hypertension,diabetes,ECOG score,adjuvant therapy,and il-6 level were not independent factors affecting the prognosis.T stage(T3(HR=56.551,95%ci :4.579-698.379),T4(HR= 332.333,95%ci:22.810-4841.998))And M staging(HR=4.805,95%ci: 1.518-15.207)were independent factors affecting the prognosis of patients.Conclusion 1.The total survival time of patients with high serum il-6 level before bladder radical resection was significantly reduced compared with those with low serum il-6 level.The high serum il-6 level before surgery indicated poor prognosis,which could be used as the basis for judging the prognosis and postoperative adjuvant treatment.2.T stage and M stage are independent risk factors affecting the prognosis of patients with radical bladder resection,while peripheral blood il-6 level is not an independent factor affecting the prognosis. |
