Objective: Matrix metalloproteinase(MMP)plays an pivotal role in the development of atherosclerosis.Recent animal experiments have confirmed that MMP-17 is involved in the formation of atherosclerosis. However, the serum MMP-17 level in patients with acute cerebral infarction has been inconclusive. Therefore, this study will explore the relationship between the serum MMP-17 level and the incidence of acute cerebral infarction, etiological classification, disease severity,short-term prognosis and carotid atherosclerosis. Methods: 119 patients diagnosed with acute cerebral infarction who visited our hospital from December 2015 to January 2017 were selected as the case group.88 patients with non-acute cerebral infarction and carotid artery ultrasound suggested carotid artery plaque and stenosis were in the atherosclerosis group.77 healthy subjects were in the control group. In the case group,51 patients who cooperated with the review on the 10 th day of the disease were in the review group.Peripheral blood of all the selected subjects was extracted within the specified time to detect the level of MMP-17 in the serum. Then we collected and organized the basic clinical information of the selected subjects for statistical analysis. Results: The serum MMP-17 level in the case group was statistically higher than that in the control and the atherosclerosis group(respectively,P<0.001,P=0.016).The serum MMP-17 level in the atherosclerosis group was significantly higher than that in the controls(P=0.047).The serum MMP-17 level in the review group was not statistically different compared with that in the case group and atherosclerosis group(respectively,P=0.93,P=1.000),but higher than that in the control group(P=0.049).Logistic regression analysis of indicators with statistical differences between the case group and the control group revealed that MMP-17 was an independent risk factor for acute cerebral infarction(OR=1.095,95%CI=1.006,1.192,P=0.035).In the case group, the serum level of MMP-17 in patients with atherosclerotic cerebral infarction was higher than that in patients with small vessel occlusion(P<0.001).The serum MMP-17 level of patients in the case group was positively correlated with thedegree of neurological impairment and the volume of infarction lesions(respectively,r=0.607,P<0.001,r=0.585,P<0.001);The serum MMP-17 level in the case group was positively correlated with the stability of carotid artery plaque and the degree of carotid artery stenosis(respectively,r=0.464,p<0.001,r=0.444,p<0.001).The difference of serum MMP-17 concentration between the case group and the review was positively correlated with the corresponding NIHSS score difference(r=0.878,P<0.001).Conclusions: The serum MMP-17 level of patients with acute cerebral infarction was significantly higher than that of healthy people. The serum MMP-17 level has the potential to evaluate the severity of acute cerebral infarction patients and predict short-term prognosis.
