Objective: To investigate the dynamic changes of serum levels of matrix metalloproteinase-9(MMP-9) and matrix metalloproteinase-2(MMP-2) in patients with acute cerebral infarction(ACI) and evaluate their clinical significance. Methods: 80 cases of ACI in the internal carotid artery system within 24 hours after the onset were recruited into this study. The blood samples were collected at days 1, 2,4, 8 and 12 after stroke onset. Serum concentrations of MMP-9 and MMP-2 were measured by enzyme-linked immunosorbent assay. 50 healthy persons were used as control subjects.Result:The serum MMP-9 levels in ACI patients were 261.7±128.4ng/ml, 382.8±151.6ng/ml, 302.7±138.4ng/ml, 238.5±121.3ng/ml and 236.6±112.9ng/ml respectively at 1d, 2d, 8d, 12d after the onset. The above serum MMP-9 levels were higher than that of the control, merely 147.1±108.6ng/ml (p<0.01, respectively). The serum MMP-2 levels in ACI patients were 367.5±253.8ng/ml, 439.2±196.3ng/ml, 441.8±178.5ng/ml respectively at Id, 2d, 4d after the onset. The above serum MMP-2 levels were lower than that of the control level 631.2±301.7ng/ml (p<0.01, respectively). MMP-9 level in the large infarction group was higher than that in the medium or the small infarction group (F=14.67, p<0.001) at 2d after stroke onset. There were no significant difference in MMP-2 levels among the large, medium and the small infarction group. The infarction size was positively linear relative to the level of MMP-9 at 2d(r=0.482, p<0.001). The decreased clinical neurological deficit scores were negatively correlated with serum MMP-9 level at 2d(r=-0.536, p<0.001). Patients with bad prognosis had higher MMP-9 level than those with good prognosis at all time points post stroke. The WBC count was positively linear relative to the level of MMP-9 at 2d after stroke onset (r=0.432, p<0.001).Gone I us i on: The MMP-9 level is elevated in the early stage of cerebral infarction, while the MMP-2 level decreased. The results suggested that MMP-9 and MMp-2 were involved in the acute phase of ischemic stroke, and the serum level of MMP-9 therefore might have a prognostic value to the cerebral ischemia.Postgraduate student: Bo Xiang(Neurology) Directed by Vice.
