Objective: To discuss the short-term effect of thoracoscopic anatomic lobectomy and segmental pulmonary resection in the treatment of early adenocarcinoma manifested as ground glass opacity. Methods:A total of 68 patients with early GGO adenocarcinoma admitted to the department of thoracic surgery of our hospital from September 2017 to December 2019 were retrospectively analyzed, including 37 patients in the lobectomy group and 31 patients in the pulmonary segmental resection group.Postoperative clinical indicators of the two groups were analyzed and compared: operative time(h),intraoperative blood loss(ml),thoracic drainage volume(ml),thoracic catheter indwelling time(h),number of lymph nodes dissected, postoperative average length of stay(d),postoperative complications, and short-term prognosis was followed up.Results:There was no statistical difference in preoperative general data between the two groups, which was comparable. In terms of operation time, the operation time in the lung segment group was slightly longer than that in the lung lobe group, but the difference was not statistically significant(P > 0.05).Intraoperative blood loss, postoperative chest drainage, and postoperative hospital stay were all higher in the lung lobe group than in the lung segment group, but the difference was not statistically significant(P > 0.05).There was no statistical difference in postoperative pain score and complications(P > 0.05).However,in terms of the number of lymph nodes dissected, the pulmonary lobe group was more than the pulmonary segment group, and the difference was statistically significant(P < 0.05).In terms of the time of chest catheter indwelling, the pulmonary lobe group was longer than the pulmonary segment group, and the difference was statistically significant(P < 0.05).Conclusion:For early ggo-type adenocarcinoma whose diameter is less than 2cm,the perioperative indicators of anatomic segmentation pulmonary resection by thoracoscopy are comparable to those of lobectomy, with positive oncology efficacy and help to protect lung function, which can be used as a preferred method.
