| Objective: To analyze the value of neutrophil lymphocyte ratio(NLR)to predict sepsis in patients with adult community-acquired pneumonia(CAP).Methods: From December 2014 to December 2018,445 adult CAP patients were selected from the respiratory and critical care medicine department of Shengjing Hospital of China Medical University.According to whether the patient meets the diagnosis standard of sepsis,Patients were divided into sepsis group and non sepsis group,including 155 cases in sepsis group and 290 cases in non sepsis group.The general data and laboratory test indexes of patients were collected for univariate analysis,and multivariate logistic regression analysis was performed for factors that had Statistical significance in univariate analysis.The area under the ROC curve was used to evaluate the value of NLR to predict sepsis in patients with adult CAP,and the best cutoff value were calculated.Results: 1.NLR level in sepsis group was significantly higher than that in non sepsis group [12.80(8.43-22.00)vs 3.89(2.10-5.90),P<0.05].2.NLR was an independent risk factor in the development of sepsis in adult CAP patients [OR= 1.15,95% CI(1.01,1.30),P < 0.05].3.The best cutoff value of NLR in predicting sepsis in adult CAP patients was 7.25,the area under ROC curve is 0.90 [95% CI(0.87,0.93),P<0.05],the Youden index was 0.67,the sensitivity was 81.9%,the specificity was 85.5%,the positive predictive value was 85.0%,the negative predictive value was 82.5%.Conclusion: NLR has a high value in predicting sepsis in adult CAP patients,and NLR is an independent risk factor in the development of sepsis in adult CAP patients.However,white blood cell count(WBC),neutrophil count(NC)and lymphocyte count(LC)are not independent risk factors of sepsis in adult CAP patients. |
