| Objectives:To understand the distribution of the traditional Chinese medicine(TCM)constitution in patients with early threatened abortion(ETA)and its correlation with TCM syndrome type,estradiol(E2)and progesterone(P),so as to provide clinical basis for improving the success rate of pregnancy with TCM conditioning before pregnancy and provide more targeted pre-pregnancy intervention for such patients.Methods:A unified questionnaire was designed by means of epidemiological investigation.According to the diagnosis and inclusion criteria,138 patients with early threatened abortion who were hospitalized in the gynecological outpatient and ward of the affiliated hospital of Chengdu University of TCM from October of 2018 to January 2020 were selected.General conditions,physical fitness scale,clinical symptoms,pregnancy-related hormone levels and color doppler ultras ound results were recorded through questionnaire survey.Sixteen patients who received estrogen or(and)progestin for fetal preservation and two patients who lost follow-up were excluded.The effective data of 120 case patients meeting the requirements were input into Excel,and SPSS22.0 statistical software was used for statistical analysis of the data.Results:1.Results of physical constitution distribution of 120 patients with ETA with unknown causes: 3 patients with mild quality,accounting for 2.5%;There were 23 cases of qi deficiency,accounting for 19.2%.There were 40 cases of Yang deficiency and quality,accounting for 33.3%.Yin deficiency in 19 cases,accounting for 15.8%;Phlegm and dampness in 3 cases,accounting for 2.5%;There were 5 cases(4.2%)of hot and humid substance.There were 12 cases o f blood stasis,accounting for 10%.Qi stagnation in 13 cases(10.8%);There were 2 cases with special report quality,accounting for 1.7%.It could be seen that the early threatened abortion with unknown reasons was mainly caused by Yang deficiency quality,qi deficiency quality and Yin deficiency quality.2.distribution of TCM syndromes in 120 patients with early threatened abortion: kidney deficiency syndrome in 40 patients(33.3%);Deficiency of spleen and kidney in 13 cases,accounting for 10.8%;Deficiency of qi and blood syndrome in 16 cases,accounting for 13.3%;Blood fever syndrome in 34 cases,accounting for 28.3%;There were 17 cases of kidney deficiency and blood stasis,accounting for 14.2%.It can be seen that early threatened abortion often witnessed kidney deficiency syndrome,blood heat syndrome.3.The TCM constitution distribution of 120 patients with early threatened abortion was statistically significant with the patient’s age,BMI,previous number of spontaneous abortion,previous history of pelvic surgery,and family history of similar abortion(P<0.05).There was no statistical significance between the distribution of TCM constitution and the patient’s normal menstrual condition,the number of days of menopause at the time of onset,and previous delivery history(P>0.05).4.The distribution of TCM constitution in 120 patients with early threatened abortion was related to the TCM syndrome type.5.The distribution of TCM constitution in 120 patients with early threatened abortion was correlated with pregnancy outcome,and the difference was statistically significant(P<0.05).6.There was no statistically significant difference between E2 and P of 120 patients with early threatened abortion who continued gestation at 5 weeks gestation and those who failed gestation(P>0.05).The difference was statistically significant(P<0.05).7.The comparison of serum E2 and P values of 120 patients with early threatened abortion at 5 weeks of pregnancy in different TCM constitutions showed statistically significant differences(P<0.05).There was no statistically significant difference in serum E2 and P values at the same gestational week from 6 to 7(P>0.05).Conclusions:(1)all the 9 constitutions of 120 patients with early threatened abortion appeared,among which Yang deficiency quality was the most,followed by qi deficiency,Yin deficiency,qi stagnation and blood stasis.Mild,phlegm-wet,moist-hot,and extra-true.(2)the distribution of syndromes in 120 patients with early thr eatened abortion was kidney deficiency > blood-heat > kidney deficiency > blood stasis > qi and blood deficiency > spleen and kidney deficiency.(3)the distribution of TCM constitution of 120 patients with early threatened abortion had some internal relations with the patient’s age,BMI,previous number of spontaneous abortion,previous history of pelvic surgery,and family history of similar medical history.(4)the constitution of 120 patients with early threatened abortion had a certain influence on the syndrome type of traditional Chinese medicine.More cases of deficiency of kidney,deficiency of spleen and kidney,deficiency of Yin and qi,deficiency of blood heat, deficiency of qi and blood,deficiency of blood and qi,deficiency of blood and kidney,deficiency of blood and blood,and deficiency of kidney and blood and stasis in blood stasis were found.(5)the TCM constitution of 120 patients with early threatened abortion had a certain effect on the outcome of pregnancy,and it also had a certain effect on the serum E and P values at 5 weeks of pregnancy.The serum E2 and P values at 6-8 weeks of pregnancy had a certain relationship with pregnancy outcome,among which the sustained increase of E2 had a higher recovery rate of threatene dabortion symptoms,and the pregnancy failure rate of those with large P fluctuations was higher. |
