Objective: The aim of this study was to identify risk factors for coronary atherosclerotic lesions in patients with type 2 diabetes mellitus,and to analyze the risk factors of severe atherosclerotic cardiovascular disease.To reduce the risk of morbidity and mortality,and provide a rationalization strategy for improve the long-term outcome of patients with type 2 diabetes mellitus.Methods: A total of 302 patients with type 2 diabetes mellitus were selected in the Third Hospital of Hebei Medical University from January 2017 to January 2018.They were divided into two groups according to with or without Coronary atherosclerotic disease(CAD): type 2 diabetes mellitus group(T2DM group)(n=192)and type 2 diabetes mellitus associated with coronary atherosclerotic disease group(T2DM+CAD group)(n=110).Clinical and biochemical characteristics were collected.The severity of coronary heart disease was assessed by Gensini scoring system.SPSS software(version 24.0)was performed for statistical analysis.Results: 1.Baseline characteristics The prevalence of hypertension and hyperlipidemia in 302 patients with type 2 diabetes mellitus was 72.18% and 55.62%,respectively.The smoking rate was 25.82%.The compliance rates of blood glucose,blood pressure and blood lipids were 24.50%,24.17% and 33.11%,and the comprehensive compliance rate was 3.31%.The prevalence of CAD was 36.42%,there was no significant difference between males and females(36.1% and 36.7%).The prevalence of CAD was increased with age and diabetes duration.Patients with three or more than three lesions was 48.18%,and those with drug-eluting stents was 77.27%.2.The clinical and biochemical characteristics between groups Age,SBP,FPG,2h PG,TC,TG,LDL-C,SCr,SUA,FIB and the proportion of patients with hypertension was significantly higher in T2DM+CAD group than those in T2 DM group.3.The results of binary logistic regression analysis Binary logistic regression analysis showed that hypertension,high LDL-C,high TG,increased 2h PG,old age,high SCr,high SBP and high TC were independent risk factors for coronary heart disease in patients with type 2 diabetes mellitus.4.The correlation between Gensini score and various indicators and biochemical parameters Gensini score was positively correlated with SBP,DBP,Hb A1 C,FPG,2h PG,LDL-C,FIB and BMI.5.The results of linear regression analysis Linear regression analysis showed that male,hypertension,high Hb A1 C,high FPG,high LDL-C,overweight/obesity were independent risk factors for the severe of coronary heart disease.Conclusions: 1.The prevalence of coronary heart disease in patients with type 2 diabetes mellitus was 36.42%,there was no significant difference between males and females.The prevalence of CAD was increased with age and diabetes duration.Patients with three or more than three lesions was 48.18%,and those with drug-eluting stents accounted for 77.27%.2.Hypertension,dyslipidemia,2h PG,old age and SCr were independent risk factors for coronary heart disease in patients with type 2 diabetes mellitus.3.Male,Hb A1 C,FPG,hypertension,LDL-C,overweight/obesity were independent risk factors for the severe of coronary heart disease.
